THIS FORM MUST BE COMPLETED PRIOR TO RECEIVING MOVE-IN INSTRUCTIONS.
Utilities MUST be transferred to your name no later than your scheduled move-in date. Not switching utilities will delay the release of keys/codes to the unit. If a utility does not apply to your property please fill N/A in the blank. Any utility bills sent to Your Place Property Management AFTER move-in date will be billed back to the tenant, along with a $75 non-compliance charge billable each week until the utility information is corrected.
