Photos from your event. Please submit a couple of photos showcasing your event. Do not submit any photos of students unless you are also submitting Photo Release forms (on our website) for each student.

If applicable: Photo Release Forms for any students in your photos.

Itemized Receipts. These need to be clear, itemized, and match your requests in your application form. Reimbursements can not be provided without detailed receipts.*

Reimbursement Name: Name on the Cheque for reimbursement (First and Last Name or School Name if school is being refunded for a purchase).*

Reimbursement Amount: The lesser of receipt total or maximum approved amount.*
