Thank you for being interested in our mini-retreat! After we gather your information here, we will email you more details and ask that you complete a brief medical screening form. If you are not a current client of ours, we will need to scheudle a complete psychiatric evaluation with you, clearing you both psychiatrically and medically for a ketamine experience. This appointment will be held with Bailie Cronin or Renee Rodgers over telehealth and can be billed to your insurance if you have Anthem, United, Cigna, or Aetna. If you do not have one of these insurance plans, there is an additional $375 fee.
