  • Welcome to Survive to Thrive Swim with Tara! I am a certified survival swim instructor through SRC network. I am also a dedicated pediatric PICU/NICU nurse of 15+ years with BLS and PALS certifications - aspiring to teach your child survival swim skills so that they can be safe around water and lead happy and healthy lives! Helping babies and children is my passion! My goal is to teach your child how to swim & float using the most efficient & effective method of teaching through sensory motor learning.

    I teach swim lessons in my own private heated salt water pool in Seven Oaks in Wesley Chapel.

    Lessons are for babies & children ages 6 months old to 5 years old.  Lessons are private and ONE ON ONE with me only and are Monday through Thursday (4x per week) for around 10 minutes each day. The swim-float-swim (or roll to float if your child is under 18 months) process takes *on average about 6 weeks of 4 days per week lessons. Each child is unique and some children may require a little longer.

    ***All scheduling is done with Coach Tara directly. Please call or text (813) 618-7902 for available time slot(s).** You will keep your scheduled time slot for the remainder of the lessons.

    Please check your vacation time, school schedule, and appointments before registering for swim lessons. 

    To reserve your time slot, the registration fee MUST be paid within 24 hours of form submission. Fee can be paid via Venmo to @Tara-SurvivalSwim, ApplePay or Zelle to 401-486-8670. Registration is NON-REFUNDABLE. Failure to receive payment may result in your timeslot(s) to be forfeited.

    Registration fee:

    New Student: $100 for 1 child, or if registering more than 1 child TOGETHER on the same form, $50 per additional child. Registration fee is valid through the end of that year.

    Returning student: Second time registration is $50 per child.  
    After the second time, registration fee is then waived (at which time a $50 per child deposit will be due which will go towards your tuition for the first week of swim).

    Registration fee is valid through the end of that year.

    Registration is a non-refundable fee that reserves your timeslot(s).

    *Registration fee does not go towards weekly tuition. Weekly tuition is completely separate. 

    Weekly Tuition:

    $100/week per child - which is due weekly during swim lessons.  

    Tuition is due the Friday BEFORE the following week.  There will be a late fee of $15 for any payments made after the Friday before the next week of lessons.  If you have not sent payment by Sunday evening, your child(ren) will not be able to swim on Monday morning. There are no exceptions.

     If for any reason you choose to forfeit lessons, the current week's tuition is still due.

  • Release of Liability: I release and hold harmless Survive to Thrive Swim with Tara, LLC, its owners, operators, and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death that may be sustained by the participants and/or the undersigned, while in or upon the premises under the control and supervision of Survive to Thrive Swim with Tara, LLC, it’s owners, operators, pool hosts, or en route to or from any of the said premises. I also acknowledge that enrollment, participation in or completion of lessons in no way implies that the participants are drown proof or water safe and that as the parent or legal guardian of the participants, I am ultimately responsible for my child’s safety in or around the water and must at all times ensure appropriate, vigilant, and persistent supervision. Swim lessons reduce risk but do not eliminate the risk of drowning.  I understand that participation in swim lessons does not guarantee water safety or drowning prevention and that constant adult supervision is always required around water. *

    Indemnification Agreement: I agree to indemnify and hold harmless Survive to Thrive Swim with Tara, LLC, its owners, instructors, and representatives from any and all claims, liabilities, damages, or expenses that may arise from my child’s participation in swim lessons.*

  • Assumption of Risk: I understand, agree and acknowledge that there are risks inherent in sports activities conducted by Survive to Thrive Swim with Tara, LLC,  and its instructors. These activities may be of hazardous nature and/or may include swimming, diving, and a variety of strenuous exercise and physical activities. With the full understanding of the facts, I state that to the best of my knowledge, the participants I am registering have no medical, physical, or emotional conditions that would hinder or prevent safe participation in Survive to Thrive Swim with Tara, LLC, Swim school.  I understand that swim lessons involve water immersion and physical activity and that, despite reasonable precautions taken by Survive to Thrive Swim with Tara, LLC and its instructors, accidents, injuries, or illness may still occur.  I understand that during swim instruction participants may occasionally swallow or inhale small amounts of water. I acknowledge that coughing, sputtering, or temporary discomfort may occur as part of learning water skills. I understand that these reactions are common in aquatic activities and do not necessarily indicate injury or unsafe instruction. I agree to monitor my child after lessons and to seek medical care if any concerning symptoms develop. *

    Child Behavior Acknowledgment: I understand that learning to swim may be physically and emotionally challenging for some children and that participants may cry, resist certain skills, or require encouragement during the learning process. I acknowledge that these reactions can be a normal part of swim instruction and I consent to reasonable instructional techniques used to help my child develop water safety skills. I understand that temporary distress does not indicate harm and that the instructor will always prioritize the safety and well-being of the participant. *

    Health and Readiness Acknowledgment: I certify that my child is physically able to participate in swim lessons and does not have any medical conditions that would make participation unsafe. I agree to inform the instructor of any medical concerns, illnesses, or developmental considerations that may affect my child’s participation in swim lessons. Children who have fever, vomiting, diarrhea, or contagious illness should not attend lessons. For comfort and safety, it is recommended that children avoid eating a full meal within 60 minutes prior to their swim lesson.*

     Medical Authorization: In the event of an injury, accident, or medical emergency involving my child during participation in swim lessons, I authorize Survive to Thrive Swim with Tara, LLC and its instructors to obtain emergency medical care for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred. *

    Supervision Acknowledgment: I understand that while reasonable care and supervision will be provided during swim lessons, Survive to Thrive Swim with Tara, LLC and its instructors cannot provide constant one-on-one supervision of participants outside the active lesson time.  A parent or guardian must remain on the property during all swim lessons. I acknowledge that parents or guardians remain responsible for supervising their child before, during, and after lessons and while on the premises. *

    Safety Rules:

    Rule 1:

    Parent/guardian must remain within visual range of the pool.

     Rule 2:

    Swim lessons are generally conducted one student at a time to ensure individualized instruction and safety. In limited circumstances, siblings may be permitted in the water together at the instructor’s discretion.

     Pool Safety Policy:

    No parent, guardian, or child may enter the pool or pool area without the instructor present and permission from the instructor.

    Pool Deck Safety:

    Running, pushing, rough play, or unsafe behavior is not permitted in the pool or pool deck area.

     Parent Supervision:

    Parents or guardians remain responsible for supervising their children at all times while on the pool premises, including before, during, and after swim lessons.

    Parents or guardians are responsible for supervising all children and guests accompanying them to the lesson location and for any damage or injury caused by their actions.

     Parent/Guardian Participation Acknowledgment:

     In certain lessons, including final or transitional lessons, parents or guardians may be invited to enter the water to observe or practice skills with their child under the instructor’s guidance. Participation by a parent or guardian in the water is voluntary.

     I understand that if I choose to enter the pool, I assume all risks associated with water activities, including slips, falls, or other injuries. I acknowledge that the instructor’s role during parent participation is instructional only and that I remain responsible for supervising and assisting my child while in the water.

    Governing Law: This agreement shall be governed by and interpreted according to the laws of the State of Florida.

  • Enrollment review and payment policies: I understand that I am enrolling for swim lessons with Survive to Thrive Swim with Tara, LLC, and that the information submitted on this enrollment form will  require review as it is the intent of Survive to Thrive Swim with Tara, LLC, to provide each participant with an individualized, effective, and safe learning experience. I acknowledge that in some health related cases, Survive to Thrive Swim with Tara, LLC, may not be able to provide services. I certify that all medical/health information I have provided is correct and true and agree to provide additional information, including a physicians clearance if requested, prior to the beginning of any lessons. I understand that in order to secure my child’s selected lesson time slot, I must pay all applicable registration fees per the instructions sent to me.

    I understand that tuition is due each week on the Friday BEFORE the following week of swim.  I agree to pay the weekly tuition fee amount that was sent to me in the confirmation email for the duration of the session that I signed up for. I understand that there will be a late fee of $15 for any payments made after the Friday before the next week of lessons.  I understand that if I have not sent payment by Sunday evening, my child(ren) will not be able to swim on Monday morning. There are no exceptions.

     I understand that if for any reason I  choose to forfeit lessons, the current week's tuition is still due.

     I understand that participant enrollment may be canceled if payment is not received. 


    Tuition reserves your child’s lesson time and instructor availability. For this reason, swim lesson tuition is non-refundable once the scheduled week of lessons has begun, regardless of attendance, illness, weather, or scheduling conflicts. Missed lessons cannot be refunded. Make-up lessons may be offered at the instructor’s discretion but are not guaranteed.

    Weather Safety Policy: For the safety of all participants, lessons may be delayed, modified, or cancelled due to weather conditions including lightning, thunderstorms, high winds, or other unsafe conditions at the instructor’s discretion. Safety decisions are final.

    By submitting payment, I acknowledge and agree to the payment and refund policies outlined above.*

     

  • Once this registration form is submitted, you agree to Venmo @Tara-SurvivalSwim or Zelle or Apple Pay to (401) 486-8670 the registration fee in the amount of $100, or $150 for new students, or $50 for returning students. **Please note NO timeslot(s) are saved until BOTH form AND fee are received.

     

    **The first weekly tuition payment is then due the FRIDAY BEFORE the Monday your child(ren) will start lessons, and then EACH Friday for the remainder of the session.
