Consent to Record and Summarise Therapy Sessions using AI

I, the undersigned, hereby grant permission to Ayna Hennessy of Safe and Sound Counselling to utilise tools that may either record therapy sessions (audio, visual, or both) or generate automated text summaries during sessions (e.g., Zoom meeting summaries, Fathom, or similar applications).

The primary purpose of these recordings and summaries is to assist with note-taking due to memory challenges, ensuring accurate and detailed session records. They are essentially a disabiliity accommodation. These notes are often shared with the client to support reflection and therapeutic progress. 

The recordings and summaries will be used for the following purposes:

Note-Taking and Memory Support: To ensure accurate records of therapy sessions and accommodate memory challenges for either the therapist or the client.
Treatment Planning: To review and refine therapeutic interventions.
Supervision: For professional supervision, if required, to ensure high-quality care.
Review and Reflection: To enhance therapeutic continuity by providing clear session records.
Case Notes and Progress Tracking: To track therapeutic progress over time.

Conditions
Limited Access:

• Audio/visual recordings and automated summaries are for the personal use of the therapist and client only.
• No other party may access these recordings or summaries without the client’s explicit written consent.

Automated Summaries:

• Tools like Zoom meeting summaries do not record or store audio/visual data. They generate text-based summaries in real time, which are securely stored and accessible only to the therapist.

Client Copies:

Notes or summaries from sessions are often shared with the client to support self-reflection and a deeper understanding of the therapy process.

Court Cases:

Recordings and summaries will not be used for legal or court-related purposes under any circumstances.

Storage and Security:

• All recordings (if made) and automated summaries will be securely stored and encrypted to ensure confidentiality.
• Audio/visual recordings will be deleted after review by the therapist unless explicitly agreed otherwise for therapeutic continuity.

Revocation:

I can revoke this consent at any time by providing written notice.

This release must be signed by the client and the therapist.
