I {name} agree to accept the risk and release Erina Ice Arena and their staff from any responsibility, cost or legal liability associated with any injuries or damages which may result from using the amenities, participating in any activity or through the actions of others within the centre.

I agree to follow any directions from the Erina Ice Arena Staff

I have read and agree to abide by the CCIHL rules. These rules can be found at https://erinaicearena.com.au/in-house-league/ and adhere to any directions of the game officials and rink staff.

I agree to wear full protective Ice Hockey equipment (including a neck guard and shoulder pads/chest protector) and any other protective measures (eg strapping tape) necessary to minimise any injury to myself during the game.

I understand that my registration will not be complete until the registration fee has been paid.

I am aware that this is a non-deliberate contact league.
