I/We the parents of the above-named candidate for a position on a Little Dribblers team hereby give my/our approval to allow the participant to participate in any and all Little Dribblers activities. I/We assume all risks and hazards incidental to such participation including transportation to and from the activities, and I/We do hereby waive, release, absolve, indemnify and agree to hold harmless the local Little Dribblers, Little Dribblers Basketball, Inc., the Organization, Sponsors, Supervisors, Participants, and Persons transporting my/our child, whether the results of negligence or for any other cause, except to the extent and in the amount covered by the League Insurance if applicable.

I/We agree, should our child be elected for All-Stars, they will be able to participate in any and all games. Should our child not be able to participate, we agree that the next child in line for the All-Star position should be placed in the position before our child. At this time, the Regional Tournament has not been set, but it is slated to be held in March 2026. Also, the National Tournament is slated to be in Franklin April 2026 at this time. These dates are set by the Little Dribblers National Office, not by the local league.   You may track the information at www.littledribblers.org; we will also keep you posted as we are notified. I/We will furnish a certified birth certificate of the above candidate to League officials.

*PEEWEE DIVISION ONLY: Evadale Little Dribblers has designed this program as a start up program for your child to help them get interested and to help develop fundamentals of basketball. We want each person to understand that in these divisions we can not get insurance to cover the children. The continental little dribblers organization does not recognize the age divisions until the children reach Juniors division. Evadale Little Dribblers is a separate organization but part of Continental Divison. By allowing your child to play you understand that they are not covered under any insurance in the event of injury.

The sign up fee this year is $100 for the first child ---$80 for the second child --$75 for each additional child in the family who wants to participate. Seniors will be $75 and will be paid at the selection of All Star teams. The sign up fee for all other divisions must be paid upon registration.
