2023 Urban Expo Vendor Form

Thank you for your interest in participating as a Vendor for the 2023 Urban Expo! This year's event will take place at the Footprint Center, home of the Phoenix Suns. Our theme is: Equity Redefined: Empowering Financial Futures in the Black Community.

We anticipate 150-200 people to be in attendance.

There is a fee to participate in the Urban Expo as a Vendor, however there are limited slots available. If you are selected to participate, you will recieve a follow up email and an invoice in the amount of $75 (retail) and $100 (food).

VENDOR AGREEMENT

The Urban Expo is a rain or shine event. Vendor fees will not be refunded outside of the stated cancellation policy below.

General Liability Insurance: All food vendors are required to have general liability insurance. Please communicate instructions to your insurance company. List Greater Phoenix Urban League Young Professionals, herein referred to as GPULYP[1] and the City of Phoenix as [2] additional insured.

Licenses and Permits: All vendors are responsible for obtaining a City of Phoenix Transaction Privilege and Use Tax License, or any and all applicable Maricopa County and /or Arizona State Licensure(s). For the City of Phoenix License call 602-262-6785. Food Vendors MUST comply with health department rules and regulations. To obtain their application for a permit to operate a temporary food establishment or have an active and valid annual license. For questions call the Maricopa County Department of Public Health at 602-506-6900.

Food Permits & Licenses: Food Vendors are serving Hot or Cold food to be consumed at or near the event. Food permits & licenses are required by vendors.

EQUIPMENT: Vendor fees include one (1) table, two (2) chairs. There is no electrical access. Vendors will have to supply their own generators if necessary.

SET UP: Vendors will be granted access to the arena for set up at 10:00am on Sunday, November 19, 2023, or unless otherwise noted. Set up must be completed by 11:30 am.

TEAR DOWN: Vendors must be able to particiapte in the entire event, and not begin breaking down their set up no sooner than 4:00pm.

INDEMNIFY: GPULYP, agents, employees, volunteers, City of Phoenix are not responsible for any loss or theft during the run of the event or set up or tear down.

CLEAN UP: Vendors are responsible for a general cleanup of their space and leave any tables and chairs provided to them in place to be counted. Failure to comply may prohibit vendors from participating in future events hosted by GPULYP.

PARKING: Food truck vendors will receive information for designated parking areas on the premises. No parking spots will be reserved for vendors. Informational vendors will receive information closer to event date on designated garage near the Footprint Center.

PHOTO RELEASE The undersigned does hereby give consent to the GPULYP and assignees to take photographs, make audio recordings and/or video graphs of myself and/or my organization at the Urban Expo and to use such photographs, audio recordings and/or video graphs for such promotion, publicity or other lawful purpose as the GPULYP deems appropriate. I/we further waive any rights of claims as to content approval or damages that I/we may have for use of these reproductions.

I have read and agree to abide by the terms and agreement of the 2023 Urban Expo and other government entities as may have jurisdiction. I hereby release and hold harmless the Greater Phoenix Urban League Young Professionals, Phoenix Suns, Footprint Center and their officers, employees, and agents from all liability, including but not limited to statues, ordinances, and any legal authority or cause, theft, personal injury, strike, public enemy, or act of God, and d indemnify these named organizations from any damages that may arise from doing business at the Urban Expo.
