In order for you to be considered as a PTO Volunteer for any fifth grade event, you will need to be a current an active member of the Fernbrook School PTO. This includes filling out the Fernbrook PTO Family Membership form and confirmation of your $25 PTO Family Membership fee. If you have not completed this yet, please do so before submitting this form.
