Your chosen sponsor must complete the below form, and give it to their pastor for his signature and parish seal. Please make sure you notify them in a timely manner, so they have enough time to get it back to us. This form should be returned to Chris Fortin by October 16th.

Confirmation Prep Session Schedule

Sessions will be held in-person in the OLMM Church Hall on Tuesdays from 6:00-7:30. The dates for Confirmation Prep are October 3, 10, 17 and 24.
Important Notes

If you do not have copies of your Baptism and/or First Eucharist certicates, please obtain copies of both and either mail them to the Parish Office or email them to Chris Fortin - cfortin@olmmparish.org.

As long as all pertinent forms and information are submitted in a timely manner, and all four sessions are attended, you will be prepared to receive the Sacrament of Confirmation. The Diocesan Celebration of Confirmation will be held on Sunday, November 5th at 2:00pm. The time of the Mass has yet to be announced.
