Please read the following and sign the agreement below. This includes all people who woud like to use the fishing pole, referred to as Third Party Users. Remeber all third party users must be 10+.

I am fully aware that fishing, especially in saltwater, can be a dangerous sport with the potential for serious or life threatening injuries occurring from the improper use of equipment, encounters with live animals, accidents, or equipment failure. I confirm that I have the necessary training and experience to properly use fishing equipment, the physical capabilities necessary to fish safely, can assess risky situations and will avoid them, will use proper safety equipment while fishing, and will immediately halt the use of damaged equipment. My signature below confirms I am representing myself as able and knowledgeable in the use of the rental equipment.

I also confirm I have all current fishing licenses required to fish in Florida, and will abide by local rules and regulations while fishing. I agree that my failure to comply and my intention to fail to comply with laws, regulations, the rules of this agreement, or any of Keys Maritime's employee’s or agent's directives will result in immediate termination of the rental at the time of the incident or intention, even if an Employee or Agent of Keys Maritime Charter's Specialty Bait and Seafood LLC was not aware.

I understand that the use or intended use of the equipment by any person not listed on the rental form at the time of rental commencement, or any misrepresentation of my own abilities unintentional or otherwise, will result in immediate termination of the rental at the time of the incident or even a failure to commence the rental, even if an Employee or Agent of Keys Maritime Charter's Specialty Bait and Seafood LLC was not aware. I agree that a Keys Maritime Employee or agent does not have to be aware that the rental agreement has been broken for a rental to be terminated or fail to commence, and that a Keys Maritime Employee or agent may make legal alterations to this agreement via a written ammendment and an attempt to contact the renter using the contact information provided by the renter. This agreement is the only rental agreement, and constitutes the only contract regardless of verbal contracts made with any agent or employee of Keys Maritime Charter's Specialty Bait and Seafood LLC. This is not the end of the agreement.

• NO TAKING RODS IN WATER! NO WADING! NO DUNKING OR SPLASHING EQUIPMENTWITH SALT WATER. OR YOU BUY A RUINED REEL!!
• THE RENTAL WILL COMMENCE UPON DELIVERY OF ITEMS TO THE RENTER, NOT UPON SIGNATURE OF THE FORMS

The following will populate the equipment rental portion. If you do not know which rod you will be using yet, attach a description of the damages when you email in this form to fishingpolerental@gmail.com, or print it out and complete it at the shop

I understand that any damage to the rental equipment deemed excessive cosmetically by an employee, that requires maintenance, is prohibitive of the equipment’s proper use, or is considered to be out of the ordinary by an employee or agent of Keys Maritime Charter’s Specialty Bait and Seafood LLC will result in the purchase of the rental equipment or incursion of the fees below. I understand any loss of line deemed large enough by an employee shall also result in the fees listed below. 

Furthermore, I agree to the proper use of this equipment as intended by the law and regulations, the equipment’s owner Keys Maritime Charters Specialty Bait and Seafood LLC, or the manufacturer. I understand any use of this equipment in a way that would violate a law, rule, or regulation, or in a way other than intended by its owner or manufacturer shall result in immediate termination of the rental agreement, and will constitute an activation of the agreement of purchase. I understand and agree that the use of this equipment by any person not listed on the Equipment Rental Form shall result in immediate termination of the rental agreement, and will constitute an activation of the agreement of purchase. I understand that this charge or charges may not come immediately, and may come from a different account.
