Calvary Youth Participant Information and Permission Form

Please fill out a separate form for each Jr. or Sr. Youth participant.

If you are over 18 you are welcome to fill out a form for yourself.

In choosing to participate in Calvary Youth events, I commit to the following: 

• I will respect others and participate in activities in a non-violent way
• I will use speech that is non-offensive and respectful to others 
• I will refrain from sexual or sexualized relationships with other participants 
• I will refrain from the use of alcohol and non-prescription drugs 
• If I am using medications, I will only take them in the manner directed 
• I will speak up and inform a leader if I feel that my safety or well-being is threatened 
• I understand that if I break these commitments, I may be sent home prior to the end of the event. 

Media Consent

We are excited about what God is doing in our midst at Calvary Youth and because of this we like to share stories and experiences with our congregation and larger community. We know that pictures and videos are a vital method in sharing what is happening at Youth Group. To be able to share specific images (clearly identifiable- not a large group shot) and videos of youth 17 and under, we require informed consent of the parent/guardian. This may include photos or videos for our congregation/community, posting on our social media (@sryouth is a private group)or for our church webpage.

At any time you may change your mind. Should that happen, please immediately notify Rhonda Elgersma, Youth Ministry Director (rhonda@calvaryunited.com).

Parental Agreement & Permission for Onsite and Offsite Activities

I give permission for my youth to participate in the activities (both onsite and offsite) of Calvary United Church Youth Group. I acknowledge that my child is participating in the youth group events voluntarily and that Calvary United will not assume responsibility for injury, loss or damage sustained by participants during the course of these activities. (Please note: on some occasions, carpooling and transportation may be provided.) In the event of an emergency I authorize staff to seek emergency medical treatment.

I understand that participating in these events may involve certain risks, such as injury or damage to personal property. I hereby release and hold harmless Calvary United Church and its leaders, volunteers, and employees from any and all liability, claims, and causes of action arising out of or related to my child's participation in these events.
