I hereby authorize the "monitoring station" to verify or attempt to verify alarm signals received by it from the premises before dispatching emergency services. The "monitoring station" shall incur no liability for any loss, damage or personal injury sustained as a result of any such verification or attempted verification.

ACCEPTANCE: I accept the above noted agreement, and acknowledge that I have read and that I understand all the terms and conditions, including limitations of liability and limitations and warranty and further, that by signing hereunder, this becomes a binding contract. You authorized to do the work as specified. If the customer is a corporation, the undersigned has the authority to bind the corporation.
