Withdrawals/Refund: If the student withdraws from the program, he/she has the option to receive either class credits towards future sessions (excluding any classes already taken at the time of withdraw), or he/she may obtain a tuition refund for the amount due at registration minus a $50 processing fee and any classes taken. Refund requests must be made in writing. No refunds are given for events that out of our control (flood, fire, electrical outage, etc No credits or refunds will be given for missing the class.

Bounced Check: The drawer, who writes the check, will be responsible for the bank processing fee

I request that the applicant listed above be permitted to participate in Outshine education center program(s I affirm that the applicant listed above is at program(s) he/she may receive necessary first aid, medical attention by a licensed physician or be admitted to a hospital in case of emergency. I will NOT hold Outshine education center, its officers, employees/staffs, agents, contractors and volunteers liable for medical aid rendered and will reimburse Outshine school. for medical or other expenses incurred in his/her care. I agree to release, indemnify, defend and hold Outshine Education Center, its officers, employees, agents, contractors and volunteers harmless and free from any and all liability resulting directly or indirectly from participation in the(se) program(s), including but not limit to liability for any and all demands, damages, claims, suits, liens and judgments, including costs and attorneys' fees, of whatever nature, for injury or death of any person, damage to property, or interference with the use of property, arising from or in connection with participation in the program(s I have carefully read this Indemnity, Release, Waiver and Authorization for Emergency Medical Care Agreement and fully understand its contents and understand that it shall be binding upon me, my heirs, successors and assigns. I am aware that this is a full release of liability and sign it of my own free will.

Photo and Video: I give permission for my /my child's (if student is under 18) photograph to be used by Outshine Education Center on its website and for any Outshine Education Center-related publicity, including print and broadcast media.

1. I understand that the weekly tuition is due on Monday of each week. I understand that the monthly tuition is due by the 1st of each month a late fee of $25.00 will be charged to my account if tuition is not received by Monday (for weekly) and by the 10th (for monthly

I understand that Outshine Education Center reserves the right to (a) close the school, revise the school calendar, and determine instructional times, schedules, and methodologies and (b) determine staffing needs and modify faculty and staff schedules and responsibilities and/or terminate its services.

3. I understand that it is my responsibility to notify Outshine office staff upon arrival if my child is on medication or has received immunizations or allergy tests within the last 24 hours.

4. I understand that I am required by California State Law to sign my child in and out of school each day. Outshine hours of operation are 8:30am-6:00pm.

5. I understand that Outshine after school does not dispense any medications (except asthma inhalers & EpiPens).

6. I understand that all fees are nonrefundable.

7. If you choose to withdraw your child from Outshine after school and want to guarantee your child's enrollment for a Summer or Fall start date, you may pay a Guarantee Placement Fee $150 nonrefundable Registration Fee (if applicable), Materials Fee (if applicable), and first month of tuition. GPF is nonrefundable and not credited towards tuition.

8. I understand that all payments returned by the bank will be charged a $25.00 processing fee.

9. I understand that Outshine will be closed on the following dates for the 2022-2023 school year:Labor Day 9/6, Veteran’s Day 11/11-11/12, Thanksgiving 11/25-11/26, Winter Break 12/23-1/2, MLK Day 1/17, President’s Day 2/21, Winter In-Service 2/22, Spring In-Service 4/04,Memorial Day 5/30, Summer In-Service 6/8-6/10, Independence Day 7/4, Fall In-Service 8/10-8/12. 

10. The full tuition payment (whether weekly or monthly) is due and payable regardless of thenumber of days attended, number of weeks in the month, or absence for any reason. Childrenwho miss a scheduled day may not then attend school on an unscheduled day. 

11. I understand that Outshine after school is a “peanut free zone.” 

12. I understand that Outshine encourages students to play outside and participate in art/park anddoes not take responsibility for stained/damaged clothes and shoes. Please dress your childappropriately to play.

13. I understand that my child will need to bring rain boots, a rain jacket, and rain pants on rainydays. 

14. I understand in order to protect the safety and wellbeing of children, staff and parents atOutshine, if my child has any of the following conditions, situations, or symptoms he or she is notallowed on the Outshine Campus: temperature over 100.4 degrees, diarrhea, vomiting, jaundice, sore throat with fever, shortness of breath, dry cough, chills, repeated. shaking with chills,headache.

15. I understand that, in general, no credit or refund is given for missed lessons. However, if youmiss a lesson that Outshine approves (one on one only) or your child is sick and is absent fromtheir school on the same day, you may ask Outshine for approval to make up that lesson.

16. I understand that NO CREDIT will be given to you when picking your children up early or when arriving late.
