THIS IS A RELEASE OF LIABILITY, READ BEFORE SIGNING.

This agreement releases Glitter & Glitz Kids Spa R LLC, from all liability relating to injuries that may occur on site and/or during one of the events. By signing this agreement, I agree to hold Glitter & Glitz Kids Spa Boutique  LLC entirely free from any liability, including financial responsibility for injuries, regardless of whether injuries are cause by negligence.

I also acknowledge the risks involved in this activity. These include but are not limited to fall, skin irritations, food allergy reaction . I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.

I acknowledge that I am at my own risk , allowed to leave my child with Glitter & Glitz staff for services, to shop in the mall.Also I acknowlegde that I may not leave off mall premises and must return within the alloted time of my child's appointment or service to pick up child or a fee of $5 per 10 minuteschild is left with Glitter & Glitz Kids staff will be charged.

 

I agree to pay all monies for services or parties rendered and agree to not dispute charges after services are rendered.

I acknowledge that I am allowed a 15 minute grace period for a spa appointment or party. After time has expired I understand that appointment or party may be cancelled or rescheduled and deposits and all payments are non refundable.

I also agree to reimburse Glitter & Glitz Kids Spa Boutique LLC for damages I inflict on the property or any material goods owned by Glitter intentionally or un-intentionally during the course of the event, at all cost deemed appropriate by Gltter & Glitz Kids Spa Boutique LLC., owners or staff. I also acknowledge that Glitter & Glitz Kids Spa LLC, staff reserves the right to refuse anyone for any reason, and at any time during the event, without refunding any portion of the cost of your service.

I also acknowledge that spa services rendered are for entertainment purposes only and services are not performed y licensed cosmetoligists.

By signing below, I forfeit all right to bring a suit against Glitter & Glitz Kids Spa Boutique  LLC for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.
