    Calif Uniform Statutory POA and Health Care Directive
  • The California Uniform Statutory Power of Attorney form (Probate Code §4401) allows the principal to grant an agent authority to conduct transactions involving financial decision making. You may name more than one agent

    Attached below are the POAs (Financial and Health Care Directives) that I carry.
    After the notary assignment of all signatures and I notarize the POAs. You will have to keep the originals and put it away in a safe place. The only time you will need them is when you have to speak to someone about the Principle needs and request. You will need to produce the originals and they need to see them to make sure that you aare the Agent and you are and will be speaking for the Principle behalf.
    Make photo copies of the original POAs and make sure that you keep the original separate from the copies. If you give away or lose your original POA, you will have to start over again and get a new POA.

    The Individual signing the POAs (Financial and/or Health Care Directive) must be coherent and must know what he or she is signing. If the Individual is suffering from dementia and cannot sign or put an “X” by themselves. Notary signing cannot happen. I request this respectfully, please let me know if this applies to you or to the person(s) signing legal document.

    If the signer is coherent and knows what's going on and can only make an "X" on the document. There will need to be two witnesses at the time of notary who can witness the "X" being done by the signer. I do not want to waste your time and my time if I’m unable to conduct notary service.


    State of California prohibits Notaries from certifying a signers status as a representative. 
    You as the "AGENT" "ATTORNEY IN FACT", cannot sign the POA on their behalf. 

  • You have the right to give instructions about your own health care. You also have the right to name someone else to make health care decisions for you. This form lets you do either or both of these things. It also lets you express your wishes regarding donation of organs and the designation of your primary physician.

  • Should be Empty:
