- I give consent for my veterinary practice to share relevant clinical history for the aforementioned animal with Wales and West Veterinary Physiotherapy
- I give consent for Wales and West Physiotherapy to discuss treatment and any concerns with my veterinary practice
- This referral form is valid for up to 12 months unless my vet advises otherwise
- Should a new or previously undiagnosed pathology be suspected at the time of assessment, Wales and West Physiotherapy may refuse treatment until the animal has been seen by a veterinarian
- Payment is due either before or on the day of treatment
- Cancellations made within 48 hours are subject to 50% payment and within 24 hours, 100% payment.
