I give permission for the above-named participant(s) to meet for ministry related activites and to commute to and from Vancouver Chinese Baptist Church (7474 Culloden St, Vancouver) in the locations below:
- Ross Park (7402 Ross Street, Vancouver Bc V5x 4b7)
- Moberly Park (404 51st Ave E, Vancouver V5X 1C7)
- Walter Moberly Elementary (1000 East 59th Avenue Vancouver, V5X 1Y7)
- McDonalds Restaurant (7289 Knight St., Vancouver BC V5P 2W9)
- VCBC Member's Home (7437 Culloden St, Vancouver, BC V5X 4K3)
This permission is valid for EDGe Youth Fellowship or CYF Youth Fellowship's operating hours (Fridays 7:30 pm - 9:30 pm, between September 1, 2023 and August 31, 2024 inclusive) and Youth Sunday Life Groups operating hours (Sundays 11:15am - 12:30pm, between September 1, 2023 and August 31, 2024 inclusive).

I understand and agree that the church and its volunteers (Vancouver Chinese Baptist Church) will not be liable for any injuries that may incur during the ministry events held in the locations mentioned above. I will ensure that my child will be on his/her best behavior and will follow all rules set forth by the leaders and the church.

Worship Service Media Release: I, the parent or guardian of the above-named participant(s), agree to permit reasonable use of photos and videos of participant taken during VCBC's ministry events (i.e. Sunday Service & Praise Nights) as part of their regular livestreaming and recorded service operations. (Please note that livestreaming and video recordings of your children may be made recorded during weeks in which the youth serves in worship team on duty. Declining this release may result in special arrangement to your child to prevent him/her from being recorded)*
