  • The information needs to be filled before the commencement of the course. This records that student has understood Lead College policies and procedures. The document will be recorded in student profile

  • Section 1- Unique student Identifier

    Unique Student Identifier is your individual education for life. It also maintains an online record of your Australian training achievements.

    You need a USI if you are:

    a student studying nationally recognized Vocational Education and Training (VET)
    a higher education student seeking a HELP loan or Commonwealth Supported Place
    a higher education student graduating in 2023 or beyond.

  • Section 2-Photography and Video Consent

    Please note that LEAD College collects your address, telephone and email details in order to obtain your consent to the publication of a photograph/media file or any other such material containing your image. These contact details will not be published or disclosed without your consent, unless authorized or required by law. Your consent is obtained through this form.

  • Section 3- Student Code of Conduct

    It applies to use of the Lead College’s resources, networks, education and support services, whilst in class, undertaking on-line activities, vocational placement activities, traineeships, apprenticeships, or any other academic activity associated with the organisation.

    Students are expected to, at all times:
    • Act in accordance with the Student Code of Conduct.
      Comply with all Commonwealth, State and Territory legislation and regulatory requirements.
    • Read and comply with the organisation’s policies and procedures.
      Meet all course requirements to the best of their abilities relevant to the education and training are undertaken with the Lead College, which includes regular attendance and engagement in learning, academic activities, and meeting the course assessment timelines.
    • Treat all staff and students with courtesy, respect, and dignity.
      Avoid interfering, or disrupting any training, teaching, learning, assessment, or any other academic activity of the Lead College.
    • Use all learning and support resources and facilities, including IT resources in a manner that does not impede learning or the learning of other students.
      Conduct themselves in a professional manner whilst undertaking a vocational placement or other forms of industry-related training and assessment activities.
    • Ensure that the Lead College’s reputation is not adversely affected.
      Comply with all reasonable directions given by the Lead College’s staff and trainers/assessors.

    This feedback form is used to gather information from students who have used an Agent to assist them with their enrolment in a Lead College course. Your feedback helps us to ensure that Agents have and are providing valued assistance to students and that the experiences and services you will receive from Lead College are properly explained to you by the Agent. Please circle the response for each question that represents your experience. If you did not use an Agent - do not complete the form.
