Forever Fido Rescue and Rescue Rehabilitation Volunteer Agreement

In signing this Agreement I understand and agree to the following:

• I will treat all animals, people, property I come in contact with at/through Forever Fido Puppy Rescue with respect.

• When I am no longer able to volunteer, I will notify Forever Fido Puppy Rescue and return any Forever Fido Puppy Rescue materials or resources.

• I agree that it is my sole responsibility to ask for further instructions or clarification if there is a matter regarding my volunteer work which I do not understand, and to ask for assistance if I am not comfortable performing a task, or not able.

• I give Forever Fido Puppy Rescue the right and permission to use my name, likeness, and voice, together with my endorsement or testimonial (whether written or oral), in all types of advertising and promotion related to Forever Fido Puppy Rescue activities. This right and permission includes photographs, video recordings, audio recordings, and all other media in which my name, likeness, or voice may be reproduced.

• Forever Fido Puppy Rescue will own any materials I prepare or make that are related to my job, whether or not made on my own time, or in a volunteer capacity. Forever Fido Puppy Rescue will also own any materials I prepare or make during donated volunteer hours, or using Forever Fido Puppy Rescue resources or facilities.

• I understand that as a Volunteer I may gain access to information about Forever Fido Puppy Rescue customers, or staff that is confidential. I agree to maintain confidentiality and to refuse disclosure of any information that is either private or personal. 

• Volunteering at Forever Fido Puppy Rescue is at-will. Active volunteer status at Forever Fido Puppy Rescue may be terminated for any reason, with or without cause or notice, at any time by either the Volunteer or Forever Fido Puppy Rescue. I understand that if I have no reported hours for six (6) months, my active status will be terminated. To return as a volunteer after more than six (6) months, I may be asked to attend orientation again, before my active status is reinstated.

Forever Fido Rescue and Rescue Rehabilitation Volunteer Liability Waiver

I have read, understand and agree to abide by the conditions of Forever Fido Puppy Rescue volunteer agreement and waiver. I understand that all work done with Forever Fido Puppy Rescue is done at my own risk and here by release Forever Fido Puppy Rescue of any liability, property damages and medical costs arising from any known, unknown or any anticipated or unanticipated damages to people, animals, or property while the animal(s) is/are under my care and supervision. I am fully aware of, understand, and agree to all terms and conditions of this volunteer agreement and waiver, and that it is a binding contract.

Release of Liability:
I understand that this is at my own risk and I hereby forever release, discharge, and agree to hold harmless and indemnify Forever Fido Rescue and Rescue Rehabilitation from all claims, demands, actions, causes of action, or liability of any king whatsoever arising as a result of doing volunteer work with Forever Fido Puppy Rescue.

I further agree that I will not make any claims against, sue, attach the property of, or prosecute Forever Fido Puppy Rescue (or any of its volunteers) for any death, personal injury, or property damage, whatever the cause or place of the event giving rise to the claim, which I may suffer or sustain as a result of or in a connection with my participation as a volunteer.

I further understand that the behavior of domestic animals is sometimes unpredictable and that some domestic animals are capable if inflicting serious personal injury or death, as well as extensive property damage. Knowing the risks of handling domestic animals; nevertheless, I hereby agree to assume those risks and to release, indemnify and hold harmless Forever Fido Puppy Rescue and volunteers who might otherwise be liable to me or my heirs or assigns for damages.

I further understand that all records, files, forms, applications, mail lists, passwords, security codes, correspondence, messages, or any other information (collectively referred to as “Information”) belonging to Forever Fido Puppy Rescue and/or bearing its logo and/or name, are the sole property of Forever Fido Puppy Rescue. Volunteer acknowledges Volunteer Agreement and will not disseminate, use, publish or sell any Information without the written consent of the Board of Directors of Forever Fido Puppy Rescue.

My participation with Forever Fido Puppy Rescue is strictly on a volunteer basis, therefore no insurance against bodily harm or damage of property is provided for me.

I have read this waiver and volunteer agreement carefully and fully understand its contents. I am aware that this is an indemnity, waiver, and release of liability and a contract between Forever Fido Puppy Rescue and myself, and by signing below, I sign it of my own free will.
