Evangel Church Volunteer Application

List friends or business associates (not relatives) who know you well that the church may contact. If you have one pastoral reference from a previous church, that would be an asset as well.

Please read the following information carefully before signing this application. I understand that information given may be verified by Evangel. I give Evangel Church the right to contact my references and do hereby release Evangel Church and its representatives from liability for seeking such information and all other persons, corporations, or organizations for disclosing such information. I certify that the information provided on this form is correct and complete to the best of my knowledge and I have not withheld any information that would unfavourably affect my application for a volunteer position. I agree to protect the privacy and confidentiality of all personal information I become aware of through my volunteer service at Evangel. I agree to respect the authority of Evangel's church leadership, to uphold the church's mission and values, and agree to not do anything to undermine the mission of Evangel. I will do my best to demonstrate teamwork, dependability, safety, integrity, and kindness in my volunteer service. I understand that different ministries require varying levels of commitment and accountability and I agree that I will sign a ministry covenant that is appropriate and coordinates with respective ministries.
