I hereby release Southeast Institute of Biblical Studies and the Karns Church of Christ, including add staff, officers, and members from any liability regarding injury, illness, or property damage to me, my family, and/or my/our property that occurs during any school or church activity, including recreational and/or social activites, whether organized or not, and whether on the school/church premises or not. 

I understand that I am responsible for determining for myself whether I am in suitable physical condition for such participation, and that I am responsible for determining this for members of my family.

NOTE: Those who have not signed and turned in this form will be asked not to use the basketball, volleyball, or other physical activity recreational facilities.
