Participation Waiver

I am voluntarily participating in the LIFT fitness class at Vail Christian Church, and I am participating in this class entirely at my own risk. I understand the instructors for this course are not fitness professionals. I am aware of the risks associated with participating in this activity, which may include but are not limited to physical or psychological injury, pain, suffering, illness, and death. I understand that these injuries or outcomes may arise from my own or others' activity or from conditions at the activity location. Nonetheless, I assume all related risks, both known
and unknown to me, of my participation in this activity.

I hereby certify that I / we are in good condition and am able to safely participate in this activity. I will additionally permit the use of my / our pictures in social media, promotional videos, sunday morning fliers, etc. As a participant, I certify that all information provided on this form is true and complete.

This waiver and release of liability shall remain in effect for the duration of my participation in the activity, during this initial and all subsequent events of participation.

IF PARTICIPANT OR ADDITIONAL PARTICIPANTS ARE UNDER THE AGE OF 18: This is to certify that my son/daughter has my permission to participate in the LIFT fitness class and is in good physical condition.
