The Wellness Nexus Program (WNP) is dedicated to enhancing the holistic well-being of Black individuals by providing resources and referrals focusing on: physical wellness, mental wellness, spiritual wellness, and social wellness. Our program envisions a community where individuals thrive in every aspect of their lives. This work will be done through thoughtful, intentional case management support for individuals seeking support from the WNP team. In this work, we are committed to equity, inclusivity, integrity, and adaptability to best serve our community.
The purpose of the WNP intake form is to gather data and qualitative information from prospective participants seeking support. This phase is dedicated to collecting essential demographic and personal information related to the prospective participant with the following objectives: (1) assess the person's eligibility for participation in the WNP program, (2) identify the participant's short-term, ongoing, and long-term needs for health, human, and social services, (3) determine the scope of support that the WNP team can provide to the participant, and (4) pinpoint prospective goals and benefits arising from the participant's engagement.

Collection of Personal Information: We collect personal information necessary for the purpose of the Wellness Nexus Program. This information may include, but is not limited to, your name, contact details, health history, and other relevant details.
Use of Personal Information: The information collected will be used for the sole purpose of providing your wraparound services. Your information will not be used for any other purpose without your explicit consent.
Confidentiality: Your personal information will be treated with the utmost confidentiality. It will only be accessed by authorized personnel who require the information for the specified purpose. We have implemented security measures to protect against unauthorized access, disclosure, alteration, and destruction of your information.
Disclosure of Information: We do not disclose your personal information to third parties without your explicit consent, except as required by law or in situations where disclosure is necessary to protect your safety or the safety of others.
Retention of Information: Your personal information will be retained for a period necessary to fulfill the purpose for which it was collected, and in accordance with applicable laws and regulations. After this period, your information will be securely destroyed.
