    • Pursuant to section 3319.39 of the Ohio Revised Code, any offer of employment will be contingent on the results of a criminal background check conducted by the Ohio Bureau of Criminal Identification (BCI). Any conviction or plea of guilty to the offenses listed in section 3319.39 (B) (1) of the Ohio Revised Code may bar the applicant’s employment with the district.

      Any person who knowingly makes a false statement is guilty of falsification under section 2921.13 of the revised code, which is a misdemeanor of the first degree

    • I acknowledge being informed that, as a precondition to employment, in the position for which I am applying, I must in accordance with Ohio law submit to fingerprinting, and satisfactorily pass a criminal records check, if I come Under Final Consideration for employment.

      I recognize that the Board of Education will pay $22.00 for the cost of the Ohio Bureau of Criminal Identification and Investigation (BCII) records check and $25.25 for a nationwide FBI records check. Unless I submit to the background check, I will not be employed by Miami Trace.

      I authorize investigation of all matters contained in my application for employment;
      certify that all statements made by me of the Miami Trace Local School District employment application are true and accurate to the best of my knowledge. In the event it is necessary for the Board to employ an individual prior to having received the results of the criminal records investigation, that individual’s employment shall be contingent upon subsequent receipt by the Board of a report from the Bureau of Criminal Identification and Investigation (BCII) which is not inconsistent with the employee’s answers on the job application. If a report is subsequently received from the BCII which is not consistent with the employee’s answers to questions on the job application, the employee shall immediately be released, and the action of the Board employing such individual shall be void without any further act by any party, and without the necessity of proceedings under R.C.3319.16 to formally terminate such employee’s contract of employment.

      I authorize all persons, schools, companies, corporations, former employers, and law enforcement agencies to supply any lawful information concerning my background and release them from all liability and responsibility arising from their doing so.

      I hereby authorize the Miami Trace Board of Education or its agents to conduct such investigations and to obtain such records (including criminal records) as the Board deems necessary and release all parties from all liability for any damage that may result from furnishing such information to Miami Trace Schools. I understand that giving false or misleading information, either oral or written, may result in denial or termination of my employment.

      I also agree and acknowledge that, if hired, federal law requires me to provide Miami Trace Schools with appropriate identity and employment eligibility documents within three (3) days of hire; and if hired, thereafter when necessary to demonstrate continued compliance with the immigration laws. I acknowledge that, if hired, failure on my part to provide such documentation will result in termination.

      I understand that Ohio public records laws may mandate disclosure of applicant and employee information by the school district. I agree to accept the provisions of the appropriate school employees, retirement system of Ohio and to comply with all rules and regulations of the Miami Trace Local School District. I understand that this is an application for employment and that no offer of employment is being made at this time.
