This form is for any guest attending class with AMDS! Guest must be age appropriate for the class, and no experience is required! Students who bring a guest will be entered into an AMDS raffle for $100 to our Pro Shop, this also includes purchases of our upcoming fall merch line!

Anna Marie Dance Studio a/k/a (Saggione Studios Inc / Saggione Holdings LLC) hereinafter Anns Marie Dance Studio I unnderstand and agree that in consideration for (me, my child or charge) to participate in any "Dance Activity", defined as (any class, workshop, rehearsal, performance, or other activity), either at the Anna Marie Dance Studio or at any other location or venue, that: - It is acknowledged that by participating in any dance activity, there is a risk of physical injury, loss, and, in rare circumstances, death, and that I expressly assume any such risk, whether known or unknown. - it is acknowledged that by participating in any dance activity, there is a risk of possible exposure to, and or illness from, an infectious disease, including but not limited to, MERSA, SARS, influenza, infections, COVID-19, or any type of virus, and that I expressly assume any such risk, whether known or unknown. - I, (including my agents, heirs or assigns) agree to release and hold harmless Anna Marie Dance Studio, its teachers, instructors, participants, dancers, employees, agents, volunteers, staff members, and facilities from any cause of action, claims, or demands I have or may have in the future, whether known or unknown. - I, my child or charge (including any agents, heirs or assigns) will not hold Anna Marie Dance Studio liable for any personal injury death, illness or exposure to any disease, or any personal property damage, which may occur on the premises before, during or after classes. - I will not hold Anna Marie Dance Studio liable for any personal injury, death, illness or exposure to any disease, or any personal property damage, which may occur at any other location or venue, before, during or after any Dance Activity. - I represent that I understand the nature of participating in a Dance class/activity and that I, my child or charge, warrant that I, my child or charge, is qualified, in good health and in proper physical condition to participate. - I understand that I should be aware of my, and/or my child's physical limitations and agree not to exceed them. - If I am signing this waiver for a minor, I certify that I am the parent or legal guardian and have the right to act on behalf of the minor. - I have read and understood this Release and Waiver of Liability; Assumption of the Risk; and I have signed this document willingly with the intention of this being an unconditional release of Anna Marie Dance Studio for any and all liability associated with my, my child's or charge's participation in any Dance Activity; an express assumption of all risks associated with participation in any Dance Activity; in consideration for my or my child's or charge's ability to participate in any Dance activity with Anna Marie Dance Studio.*
