World's Finest Chocolate-Permission Slip

It is fundraising time! I know you all have seen the World's Finest Chocolate Everywhere! Well guess what, we decided to bring it to Natalia Rice Academy! If you would like to help us raise some funds and can sell an entire box of the World's Finest Chocolate, we want you to partner with us. Please fill out the permission slip below.
Fundraiser Guidelines

This fundraiser will be an ongoing fundraiser for several months. Families are able to choose between individual candy bars or boxes (60 pcs per box). Please make your selection below. You are always able to acquire more candy if you would like to sell more; just let us know and we will be more than happy to supply you with more.

We are requesting that all earned monies be submitted to Natalia Rice Academy on or before the of the month. Even after the fundraiser is over, you may still sell your candy, and we ask that whenever it's submitted, you place the child's name and amount sold on the envelope when submitting cash payments. If submitted via Cash App, please include the child's name, chocolate fundraiser, and the amount. Please submit all funds via Cash or Cash App $NatashaHunter0115 to Natalia Rice Academy.

Any remaining chocolate that has not been sold at the end of the fundraiser cannot be returned to Natalia Rice Academy and will not be accepted. No Exceptions.
