This eliminates most of the stress that your fur child may experience so that they can enjoy their spa day. Your fur babies will be the only ones here during their spa day. With no crating or barking dogs they will be able to relax and focus on getting pampered. (Submitting this form is not a guarantee of an appointment.)

How is your dog around other dogs?
We don't cage or kennel at our salon. Are you able to pick your dog up within 15 minutes of being called of grooming completion? (We will give you 15 minutes advance notice so you can be on your way to pick up your dog.) ***Please review our policies on our website about fees that may be incurred for late pick ups and late cancellations.
Does your furbaby have any matting or tangles? Please indicate where. (We need to know this so that we can block out extra time for your appointment.) ***Please review our policies on our website about fees that may be incurred for matted dog coats.
Please describe how you would like your dog's haircut. (Please note, the term Puppy Cut does not give us very much information. Most dog groomers interpret the term puppy cut to mean one length all over on the body, but it doesn’t specify how much length should be left and/or what to do with the dog’s head, face, ears, or tail. It could be as short as shaved to the skin or as long as 2 inches or more (We currently do not accommate hair cut lengths longer than 5/8" long, show cuts, severely matted or heavily soiled). No show cuts or no shaved downs on double coated dogs. ADDITIONAL STYLING REQUESTS AT TIME OF APPOINTMENT MAY NOT BE POSSIBLE DUE TO TIME RESTRAINT.
If you did not hear from us soon, it is because we did not have an opening. Would you like us to keep your contact information for the next 30 days so we can contact you if an opening should arise? After 30 days, your form submission will be deleted, however you can re-submit.
I understand that if my dog is very anxious, nervous, fearful, jumpy, ill, or aggressive that my dog may not be groomed or you may not receive a "perfect" haircut. (In this case, we may refer you to a grooming facility that is a better fit for you. [ ex. multiple groomers and/or veterinarian on location ) ***Your dog's safety comes first to us NO MATTER WHAT!

KEELEY IS USUALLY BOOKED 1 YEAR IN ADVANCE. Are you able to prebook your dog’s grooming appointments? Most grooming appointments are every 4-6 weeks apart. (NEW APPOINTMENTS WITH KEELEY MAY BE BEFORE OR AFTER NORMAL BUSINESS HOURS DUE TO HIGH DEMAND.)
