KCK has provided me with a rabies prevention pamphlet provided by PACFA, and KCK veterinarian/vaccination records for the pet. *

This is provided to you at the completion of the adoption. You will get a hard copy if you are at an adoption event, otherwise, you will get it electronically with the rest of the adoption documents.

I agree to pay a non-refundable adoption fee in regard to this adoption prior to the transfer of said pet to the adopter.

I acknowledge that KCK does not make any guarantees regarding breed, health, training or temperament. I acknowledge the pet is adopted in its present condition and I assume all risks and responsibilities including the financial responsibility of ownership of this pet once the pet is in my possession.

I acknowledge that the pets in the KCK adoption program are rescued and therefore, the actual age of the pet is unknown. The approximate age of the pet is estimated using our experience, any prior knowledge given to us about the pet, and the advice of veterinarians.

I acknowledge that any adopted pet needs time to adjust to its new home environment. KCK encourages adopters to consider the physical and emotional needs and requirements of a new pet and is happy to recommend a reputable trainer to help the adopter work through any potential behavioral issues.

In accordance with PACFA Rule 19.9.5, I acknowledge that Karl’s Canine Krew has provided me with all required information regarding the dog I am adopting. This includes the dog’s transfer date and origin, breed or species, identifying characteristics, date of birth (if known), microchip or other identification, vaccination and medical history, current medications and instructions (if applicable), known health or behavioral notes, and regulatory information about the Colorado Department of Agriculture’s Pet Animal Care Facilities Act (PACFA) Program. If receiving documents digitally, I understand that I will receive this information within 48 hours of adoption.

KCK pets are typically affectionate and friendly, and will not place a pet known to be of an unsound temperament, however, no one can predict how any pet will react in a given situation, especially without complete knowledge of the pet's life experiences. Therefore, KCK recommends the adopter use caution in introducing the pet to new situations (i.e., introductions to other animals, dog parks, stores, groomers, vet offices, other homes, other people and small children). I accept all responsibility and liability for all interactions with people and other animals.

I agree to contact KCK if I am having training or behavioral issues with the pet and I may need to return the pet. I agree to work with a trainer and pay for said training for a minimum of four (4) training sessions over a one month period to see if the situation can be improved and so that the pet can be evaluated by the trainer. If recommended by the trainer I agree to have blood, urine, and/or stool tests completed at a veterinarian of my choosing and expense, prior to returning the pet.

I agree to never sell, trade, or give away the pet to anyone. I will never relinquish this pet to any shelter or pound. I will notify KCK if I am unable or unwilling to keep this pet, and I will return this pet to KCK after arrangements have been made for such return. I agree to give KCK at least 7 days to find a foster for the pet prior to return.

I understand that he pet that I am adopting is a rescue pet with a largely unknown background. This pet may have come from a region of the country that has fleas, ticks, and multiple types of parasitic worms. I understand that the rescue may be unaware of infestation in the pet. But if they are aware they have treated the pet to rid it of any parasites that may be present. I understand that parasitic worms are endemic to Colorado and it is possible for the pet to become re-infested following treatment. I know the importance of heartworm and the need for testing.

I understand all future medical and health maintenance treatments are my responsibility. I will seek immediate veterinary care should this pet become ill or is injured. I understand this is for the lifetime of the pet.

I will not abuse or neglect the pet or allow it to be abused or neglected. I will not allow any physical, mental or emotional abuse of the pet.

The pet shall not participate in or be trained as a guard dog or fighting dog.

I agree to keep all immunizations current and updated according to a veterinarian's schedule and state licensing requirements.

I will not leave the pet alone with small children.

I agree to keep the pet primarily indoors, interacting with my family, and not living outdoors. I will never tie the pet to a chain or rope and/or leave the pet unattended for long periods. The pet will be exercised in a fenced yard or on a leash and will not be allowed to roam the streets.

I affirm that no member of my household has been convicted or charged with any animal welfare law violation such as hoarding, unsanitary housing conditions, neglect, cruelty, abandonment, etc. I affirm that no member of any household has been charged or convicted with any domestic battery or sex crime.

I agree to comply with all state and local ordinances in the confinement, care, vaccinations, and licensing of this pet. I will reclaim this pet promptly, and pay any required fees, if it is picked up by an animal control agency. In the event that KCK has to retrieve the pet from any pound, shelter or person that is in possession of the pet after the adopter has been notified, adopter agrees to immediately reimburse KCK for any amounts expended in procuring the pet's safe return, and KCK reserves the right in its sole discretion to determine whether the pet shall be returned to adopter or retained by KCK.

I understand that KCK shall not be held responsible for any bodily injuries or property damage and/or damage to other animals caused by the pet or by the actions of the adopter. The adopter hereby specifically assumes sole responsibility for the actions of the pet or the adopter that may result in bodily or material damages. I, as the adopter, hereby release KCK from any and all liability associated with such damages.

I understand that breach of this agreement by the adopter will result in travel costs, court costs and attorney fees payable by KCK if legal action or travel is necessary for KCK to recover the pet. I understand that pet has intrinsic value and is not replaceable, so if the pet is euthanized, lost or otherwise unable to be returned to KCK due to the negligence or intent of adopter to circumvent KCK's right to re-process the pet, I will be in breach of this agreement and will be subject to pay liquidated damages to KCK in the amount of $2,500 in addition to all attorney's fees and costs and actual damages incurred as a result of the breach.

I understand that the pet's known background and medical history will be relayed to me via email. I understand that KCK is making no representation concerning the health, condition, training, behavior, or temperament of the pet.

I understand that all information provided as part of the Adoption Application provided to KCK is true, complete, and correct and agree that any false or misleading statement thereon will constitute a breach of this contract. If any information contained on adopter's Adoption Application is untrue, adopter expressly agrees that KCK may retake custody of the pet without refunding the adoption fee and without recourse by the adopter, and adopter agrees to relinquish the pet to KCK.

I understand that this Agreement shall be governed and construed in accordance with the laws of the Sate of Colorado.

I agree that any disputes between the parties to the Agreement concerning the subject matter of the Agreement shall be submitted for resolution to the courts of El Paso County Colorado.

I understand that by voluntarily signing this Agreement I am entering into a legal and binding contract with KCK.

I understand and agree to all terms and conditions detailed within this contract and I am signing this contract myself and in good faith.
