The following information is used to create an NFA Gun Trust for your NFA firearms through the 199Trust system. Please ensure all fields are completed and everything is current and accurate.

Utah FAST is not responsible for errors on the NFA Trust if this form is not completed correctly.

Note: If you have already submitted a copy of your trust docs along with a form 1 or 4 to the ATF you will need to use the 'common amendment examples' document sent with your trust email to amend your original trust. Change request fee is $10 after 10 day inspection period.
