2022-2023 Early Intervention Grant (EIG) Final Report

Each corporation who received funds from the 2022-2023 Early Intervention Grant is required to complete this final report by 11:59 p.m. ET Friday, December 15, 2023. Indiana public school corporations must complete this report at the corporation level. Charter schools and accredited non-public schools (including choice schools and diocesan schools) must complete this report at the school level. Failure to complete this report may result in corrective action.

Personnel responsible for the use and reporting of funds must review the "2022-2023 Early Intervention Grant Final Report Guidance" before completing this final report. The guidance includes instructions for completing this form. Contact the Office of Teaching and Learning with any questions.
