    My name is Tessa Hafoka and I am the new Health Improvement Practitioner (HIP) at Mangere Health Centre.


    I’m an occupational therapist by training and enjoy supporting people to connect with what is meaningful to them, whatever that means. I have experience working in mental health and social services, alongside lived experience of disability and chronic pain.


    As a HIP, I will be providing brief sessions that focus on practical strategies and linking people with external services that support people wellbeing.

    Some of the common aspects I can support people with are:

    • Stress
    • Sleep
    • Pain
    • Anxiety, depression, or feeling down
    • Coping with physical health conditions
    • Relationship challenges
    • Drug or alcohol use

    My name is Anu Sharma and I am the Health Coach based at Mangere Health Centre. I'm of Fijian/Indian heritage. I have a Degree in Sports and Recreation and a background in Nutrition. I enjoy being part of the clinics team to support the community with their health and wellbeing.

    As a Health Coach I can provide the knowledge, skills and confidence to help you reach your health goals. Some of the common aspects I can support people with are:

    • Managing conditions like prediabetes, diabetes, asthma, gout and high cholesterol
    • Eating habits and/or exercise
    • Being smokefree or reducing smoking
    • Understanding your medications
    • Connecting with a support worker or other services

  • Awhi Ora are able to support and walk alongside tangata to help them to achieve their goals.

    Some of the common supports provided by our Awhi Ora team include:

  • When you call to make an appointment the reception team will offer you the next availale appointment.

    Our reception team will not always be able to offer you an appointment on the day you request, however, we will endevour to do the best we can to help you on the day, you may be put on our "call back" system, this means a nurse or GP will give you a call to see how they can help you.

    To help our team, please let the reception team know your current mobile/phone number. Ensure your phone is charged and turned on, our team are experiencing a lot of unanswered calls when they contact you.

  • You may have noticed that our clinical team is working a lot virtually.  What does this mean for you?

    This means the GP or nurse may do a phone or video consultation with you, this could include reviewing your results, or doing repeat prescriptions.

    Below are our virtual consultation fees, please note if you request your prescription via health365 you only pay $5.00!  If you are not on health365 please call reception on 09 2550600 and they will be happy to send you out the registration details,  this is a free service! :)

  • Recommendations if you become unwell with COVID-19 symptoms:

  •  Stay at home if you are feeling unwell - if you, or someone in your household develops one or more of the following symptoms for COVID-19 (a runny nose, sore throat, cough, fever, vomiting, diarrhoea, headache, loss of smell or taste, shortness of breath), you should take a Rapid Antigen Test (RAT). 

    Remember to register your RAT result at https://mycovidrecord.health.nz/ (or call the RAT helpline at 0800 222 478 and choose option 1), so you can be connected with any help and support you might need. 

     

    WHAT TO DO IF YOUR COVID RAT RESULT IS POSITIVE?

    • If you have tested positive for COVID-19, it is recommended you isolate for 5 days
    • Isolate even if you only have mild symptoms, starting at Day 0, which is the day your symptoms started or when you tested positive, whichever came first. This means you should not go to work or school.
    • If you do need to leave home during your recommended isolation period, it is very important you take precautions to prevent spreading COVID to others. Please don’t visit pēpi, at-risk, or elderly people if you are sick. Many illnesses are very infectious, and they can get very sick, very easily.
    • After 5 days, if your symptoms have resolved and you feel well, you can return to your normal activities. We recommend you wear a mask if you need to visit a health and disability care facility or an aged residential care facility, or you have contact with anyone at risk of getting seriously unwell with COVID for 10 days.
    • If you are currently in isolation, we recommend you continue isolating until you have completed 5 days. If you have already been isolating for 6 or 7 days, and are well, you can return to your normal activities.


    If you need to seek in-person medical care while you have COVID-19, please call ahead before visiting, and follow their instructions. It is recommended that you wear a mask.

    It is recommended that you wear a face mask when visiting healthcare services. Please respect the healthcare facility/hospital’s policy on mask wearing when visiting, you may be asked to wear a mask in particular situations or locations within a healthcare facility to help protect those at higher risk.

    -residential care facilities for older people and people with disabilities

  • All about Diabetes

    Quality Improvement Project
  • The quality improvement project for this year focuses on educating and supporting our patients manage their diabetes using the newly subsidised medications for type 2 diabetes patients who are at high risk of heart and kidney complications.

    We want our patients' lifestyles to be active, healthy, and unaffected by their diabetes.

    Over the course of the coming months, we will be contacting our patients who are eligable. As a result, you may receive a call from your GP, Nurse, Health Improvement Practitioner, or our Health Coach.
