RELEASE, INDEMNITY, WARRANTY REGISTRATION AND ASSUMPTION OF RISK

In consideration of your acceptance of this entry to the Provincial Ne-waza / Tachi-waza Judo Tournament hosted by Regina Y Judo Club, I for myself my heirs, executors, administrators and assigns, do hereby release and discharge the Saskatchewan Kodokan Black Belt Association (“Judo Saskatchewan”), Regina Y Judo Club and the officers, directors, employees, volunteers and any and all representatives of each organization from any and all claims or demands, losses, or injuries incurred or sustained by me as a result of attending, competing in, practising for or travelling to or from the 2024 Regina Y Judo Club Ne-waza / Tachi-waza Judo tournament held in Regina, Saskatchewan.
All payments will be taken at the club by cash, debit, credit, etransfer, or club credit.

ALL ATHLETES MUST CHECK IN WITH THE REGINA Y JUDO CLUB ON ARRIVAL.
