It is Clover School District policy to only allow a student to leave a school activity (away ballgame, contest, field trip, competition, concert, etc) with their parent or an adult legal guardian, no other family members, no friends, no students nor neighbors.
To avoid any confusion, any student wishing to leave an away event, (with their parent or guardian), must have this form filled out and signed electronically by the parent or guardian. The form must be submitted following their approval from Mr. Langdale or the appropriate Director in charge of the trip.
Chaperones cannot approve the student to leave an event. Forms and approvals should be made at least a week in advance of an event (or designated timeline by Director). Staff will give the chaperone's the forms so they know who is going to leave.
PLEASE NOTE: When you are ready to take the student, you must come and see the Chaperone/Director before leaving with them and let the Chaperone see you leaving together. ID would be required to prove you match the name on the request if necessary. This is mandatory so that we know the student is with the parent or adult legal quardian and is safe.
