In acceptance of this Laptop for usage, I agree to the terms and conditions stated below:

• I am responsible for the safe keeping of the laptop whilst in my possession.

• I am responsible for keeping the laptop in good condition until the time of return.

• I shall not install any program or software that is not permitted to use by the company, for privacy and security reasons.

• I am the only authorized person to have access to and use the data saved in the laptop. Any unauthorized access to this laptop is a violation of the company's policy and employment regulation and contract.

• I shall remove all data that is not company or work-related before returning the laptop to the designated department.

• In the event of loss or theft, it shall be reported to the police within 24 hours. In the event of damage, it shall be reported to the company's administration department within 3 days. A copy of the police or incident report will be submitted to the company for verification purposes.

• I understand that a violation to any of these policies is subject to disciplinary action by the company.
