2025 Ice Sculpture Order Form

Minimum Order for sculpture and luge delivery is $350.00.

**QUOTE REQUESTS:** If you are requesting a quote, you will not receive a design or no hold will be placed on the delivery schedule.

**ORDERS:** Once you have placed an order, your event will be added to the delivery schedule and entered into the artist’s design queue.

**SET UP / DELIVERY TIMES:**You will be scheduled for the time you requested on this form. If your requested time is unavailable, we will assign a time as close to your request as possible. Please remember that ice melts, so the set-up time should be no more than one hour before the sculpture will be used.

**DESIGN:** The artist will work on designs in the order of event dates. If you are placing your order well in advance, THANK YOU! However, it may take some time before you receive your proposed design.

**UPON DELIVERY:** You or your venue will need to provide the tables and linens, have someone monitor and empty the drain bucket every few hours, and dispose of the sculpture at the end of the event. Please check with your venue, event planner, or caterer to see if they are willing to take care of this for you. We do offer breakdown service for an additional fee.

**PAYMENTS:** We accept credit card payments through our online payment system, cashier’s checks and money orders. Cash payments will receive a discount when received by the due date. We do not accept personal checks. Deposits are due once a design has been sent to you. Final payment is due 10-14 days in advance depending upon design.
