Ice Sculpture Request Form
Minimum order for delivery is a full-block sculpture.
Smaller sculptures can be picked up from our shop.
**SET UP / DELIVERY TIMES:**You will be scheduled for the time you requested on this form if that time is available. Your set up time should be no more than 1 hour prior to start time.
STAIRS: If you indicate that the sculpture will be on ground level and it is actually upstairs, we may not be able to set up your sculpture and no refund will be issued.
TABLE HEIGHT - The hight of the surface on which the sculpture will be placed can be NO MORE that 40" from the floor. We cannot lift a sculpture beyone 40".
DEPOSIT: We do not require a deposit until we begin working on your design.
**DESIGN:** Once you have placed an order, The artist will work on designs in Event Date order. He will send a design approximately 30 days prior to the event date. Orders received less than 14 days prior to the event may be "Artist's Choice" based on your input.
Once you submit the form you will be emailed additional information. Please read it, print it out, and keep for your records.
You are responsible for providing a sturdy table and linens.
Standard pricing does NOT include us returning to the site to breakdown the sculpture.
