Requested Items Form

The HEROES program is pleased to meet the needs of financially insecure individuals and struggling students to ensure their retention on campus. Resource Center items are made possible by grant-funding and donations, and are subject to availability. Some items require additional approval, are subject to limitations, and/or require a Loan Agreement. If you would like to work with the HEROES liaison on the needs of your unique situation, please email heroes@siu.edu.

Please read and sign before you begin your request: 

HEROES Requested Items Policy:

1. Submit a Requested Items Form for approval from the HEROES Review Team.
1. Up to 20 items may be requested per Request Items Form.
2. One Requested Items Form may be submitted every 60 days.
2. Expect an email within two business days regarding approved items and a link to schedule an appointment.
   1. Appointments may be scheduled no sooner than two business days after approval.
   2. Appointments must be cancelled or rescheduled at least one business day prior to the appointment.
3. Lack of adherence to the Requested Items Form and Scheduling Policy may result in denial of the Requested Items Form. A new form may be submitted after 30 days.

HEROES Store Policy

1. The HEROES Store, located in the Student Center basement, is by appointment only.
1. Schedule an appointment no sooner than two business days after approval of your Requested Items Form. 
2. Appointments must be cancelled or rescheduled at least one business day prior to the appointment.
3. Arrivals more than 5 minutes late may be asked to reschedule.
4. No-shows may result in denial of the Requested Items Form. A new form may be submitted after 30 days.
2. Check-in with an ID prior to picking up or selecting approved items.
3. No more than two people will be allowed in the store at a time, accompanied by a staff person.
4. The Student Code of Conduct applies to students within the HEROES Headquarters.
5. Items are subject to availability and may not be exchanged or substituted. Certain items require additional approval from the HEROES Review Team, are subject to limitations, or require a Loan Agreement. 
6. Lack of adherence to HEROES Store Policies may result in denial of future Requested Items Forms.
