    This form is intended to help us recruit members for our Community Advisory Committee.

    What is the Community Advisory Committee?

    The community advisory committee is a group of trans, non-binary, and gender diverse people who advise and support the development of multiple aspects of Foria's services. Committee members provide feedback on subjects such as patient communication, program development, branding, etc. 

    The objective of this committee is to ensure that community members have an active role in creating our service. First and foremost, this clinic is a collaboration between our clinicians and our patients to build a service that works for you.

    At this point, our committee is only accepting those who are 16+ years old.

    Committee Responsibilities

    The committee will meet approximately once per month for a one-hour group meeting. This meeting will be facilitated so that multiple updates are provided to the group and feedback is gathered through group discussion.

    Video will not be required of participants - choose the option you’re most comfortable with. Chat participation can also be an option for folks who are not comfortable speaking up in a group setting.

    Committee members can also choose to participate in survey feedback and article revision if they are interested.

    If you would like to partake in group setting discussions but not in the survey and article reviews (and vice versa), that is an option as well. 

    Compensation

    Foria understands that participants are sharing valuable lived experiences and community expertise. As such, compensation will be provided to participants on a monthly basis through a virtual MasterCard giftcard.

    Compensation will be of $50/hour. Attending a 1-hour meeting will correspond to a $50 payment. Filling out our survey should take around 30min, and has a payment of $25. Payment for attending the meeting and filling out the survey (1.5 hours) will be $75.

    Review of articles and written content may be something we ask of committee members at some point (only to those who would be interested). The payment offered for the review of written content will be $30/article.

    Confidentiality

    During your participation in the Community Advisory Committee, committee members’ identities will be protected and not associated with your opinions and feedback, unless explicit consent is given by members.

    The moderator of the group discussions and surveys will not disclose personal information about the identity of participants throughout the project.

    This Survey

    This survey will ask you personal questions about yourself and your identity for the purposes of fair and diverse recruitment for our Community Advisory Committee. We want to ensure this committee represents the full diversity of trans, non-binary, and gender diverse communities. As such, recruitment will be focused on including a wide variety of experiences in the committee’s membership.

    The information shared here will only be used for the purposes of recruiting members for this committee and will not be shared with team members outside of this project.

    The information disclosed here is completely voluntary and you can withdraw your consent at any time by closing this survey.

    Here, we will ask you whether you're interested in participating in our Community Advisory Committee, and what capacity you can commit to.
  • Committee Responsibilities

    This section will ask you personal questions about yourself and your identity for the purposes of fair and diverse recruitment for our Community Advisory Committee. We want to ensure this committee represents the full diversity of trans, non-binary, and gender diverse people. As such, recruitment will be focused on including a wide variety of experiences in the committee’s membership.
  • Definition

    A racialized person is someone who is an actual or perceived member of a group which has been subject to discrimination and racism based on skin colour, ethnicity, religion, language or other characteristics (other than Indigenous peoples as defined in the next questions). Racism is an ideology that either directly or indirectly asserts that one group is inherently superior to others and is often deeply rooted in attitudes, values and stereotypical beliefs. Often these are unconsciously held and have become deeply embedded in systems and institutions that have evolved over time. Racism operates at a number of levels, in particular, individual, systemic and societal.

    An Indigenous person includes First Nations, Métis, and Inuit. (Note: this includes both status, treaty or registered Indigenous persons, as well as non-status and non-registered Indigenous persons.)

  • Definition

    A person with a disability has a long-term or recurring physical, mental, sensory, psychiatric and/or learning disability and considers themselves to be disadvantaged by reason of that disability, or believes that an employer or potential employer is likely to consider them to be disadvantaged in employment by reason of that disability. A person with a disability may also be someone whose functional limitations owing to their disability have been accommodated in their current workplace. Disabilities can be visible or invisible.

    What's next?
    • Our team will review your answers and make a decision about who will be included in the Community Advisory Committee

    • Decisions will be made based on the objective of having a diverse group, which includes multiple intersections of identities

    • As such, not all participants who submitted a form will be selected as part of the committee

    • We will let every respondent know via email about committee membership selection
