By submitting this form to request an in-person Chemical Safety Training Session with Milestone Chemicals Australia Pty Ltd, you agree to the following terms and conditions:

Your booking is subject to availability, and it will be confirmed via email by our team within the following days. If you have not received confirmation within 7 business days, please contact Head Office on (03) 9450 4555. Submission of this form does not guarantee a booking. If you need to cancel or reschedule your booking, please provide at least 48 hours notice to your Account Manager, or contact Milestone Chemicals Head Office to reschedule. Attendance: You are responsible for ensuring that participants arrive on time for the training session. Late arrivals may miss valuable content. Content and Materials: All training materials and content provided during the session are the intellectual property of Milestone Chemicals Australia and may not be reproduced or distributed without prior written consent. Hand outs may be provided during session. Liability: Milestone Chemicals Australia is not responsible for any loss or damage to personal property during the training session. Privacy: Personal information provided in this form will be used solely for the purpose of processing your booking and may be shared with our training team for scheduling and communication purposes. Identifying information, including name and department, provided in this form or at a later date, relating to this training session(s), will be used by Milestone Chemicals to provide a Certificate of Completion. Changes to Terms: Milestone Chemicals Australia reserves the right to modify these terms and conditions at any time. Any changes will be updated on this form and when available, on our website, and it is your responsibility to review them periodically.
