THIS AGREEMENT is made between the undersigned dog owner and (hereinafter referred to as “Owner") and Release The Leash Dog Training (hereinafter referred to as “Trainer” and/or “Release The Leash Dog Training”), and the parties hereto agree as follows
WAIVERS TERMS & CONDITIONS
***General Policy***
- Release The Leash Dog Training will endeavor to create as safe environment as possible for the training and boarding care of the dog(s) identified as (the dog), and will offer a safe, sound and responsible training, care and post-boarding training instructions.
- Owner agrees, however, that all animals may exhibit unpredictable behaviors and that Trainer will not be liable for the actions of the dog that are outside the control of Trainer
- To insure safety of our kennel facility, Owner agrees that his/her dog must be on good health condition before boarding with us. Dogs that are on deworming medications cannot be accepted into our training programs.
- Trainer agrees to provide a professional, precise, efficient, and effective dog training techniques to both Owner and Dog, using safe and humane training methodology.
- Trainer agrees to provide such training, so long as the dog is found of sound physical and emotional health and so long Owner follow's Trainer's instructions, practices regularly with his/her dog, and completes the entire course of instruction.
- Trainer makes no representation, warranty, or guaranty of improvement to dog 's behavior or demeanor and assumes no responsibility or liability for any unwanted change in demeanor or nature of the dog's behavior during or subsequent to the prescribed course of training.
- Owner understands and agrees that our trainers may employ tools they deem necessary to achieve your training goals. This may include a prong collar, slip lead, martingale, pet corrector, bark collar, muzzle, crate, and remote collar/e-collar. The trainers also may suggest dietary changes.
- Owner agrees to Trainer to obtain medical treatment for Owner’s dog(s) if he/she appears ill, injured, or exhibits any other behavior that would reasonably suggest that dog(s) may need medical treatment including anesthesia. Owner agrees to be fully responsible for the cost of any such medical treatment and for the cost of any transportation for the purposes of such treatment. Owner gives permission to Trainer to use Owner’s vet or nearest 24-hour vet hospital for required treatment. Owner agrees to reimburse Trainer for veterinary expenses Owner's dog emergency medical visits.
- All dogs must have proof of vaccinations and present either before or day of drop-off. If shots have not been updated, cannot allow dogs to be housed at the facility. Required shots rabies, bordatella flu shot and fecal float. If there is a problem with vaccinations due to reaction, please provide a note from the veterinarian.
Payment Policy: Upon signing this contract Owner agrees to put a non-refundable deposit of $250.00 to reserve a spot.
Owner agrees to pay the remaining balance the day before the drop off day.
Owner understands that their reserved spot will be lost if they failed to pay the remaining balance.
Cancellation Policy: If the Owner chooses to NOT continue the training after receiving some time of services, a refund WILL NOT be issued.
If the Owner chooses to RESCHEDULE the Board and Training Program, owner MUST notify the Trainer 1 WEEK ahead before the appointed drop-off date.
If the Trainer feels that the Owner will not listen to the training advices/follow instructions, the Trainer can at sole discretion, choose to TERMINATE the training contract and a partial refund will be issued.
Photos/Video Release
I(Owner) hereby assign and grant to Release The Leash Dog Training the right and permission to use, reproduce and distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representations(s) and/or sound recording(s) made of me and /or my dog(s) at any time during the training and/or boarding of my dog(s) by Release The Leash Dog Training and I hereby release Release The Leash Dog Training or any person that represents Release The Leash Dog Training from any and all liability resulting from such use, reproduction, distribution, and publication.
I hereby authorize the reproduction, sale, distribution, copyright, exhibit, broadcast, and/or electronic storage of any and all such photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and or sound recording(s) without limitations at the discretion of Release The Leash Dog Training and I specifically waive any right to any compensation I may have for any of the foregoing.
Physical Activity Agreement
Your dog will be staying in my home and in its crate throughout the day as needed for training purposes and not living outside. Your dog spends a good amount of time in the outside world doing lots of things around lots of natural objects, dirt, grass, cement, trees, fencing, bushes, swimming, and interacting with lots of other dogs, and humans. It is likely that your dog is going to get scratched, bumped, or bruised. Your dog will most likely get some wear and tear on him/her. Things are bound to happen even though we have taken special care in designing our facility and maintaining a high standard of safety and cleanliness. This is just like when you were a kid going out to play and would come back home with scratches or cuts, because you were having fun and exposed to the outdoors.
AGREEMENT TO HOLD HARMLESS, WAIVER AND ASSUMPTION OF RISK
Owner understand that attendance of a dog obedience training class is not without risk to myself, members of my family or guests who may attend, or my dog, because of some of the dogs to which I will be exposed may be difficult to control and may be the cause of injury even when handled with the greatest amount of care. Owner understands a muzzle may be needed to prevent possible bite, if the dog is deemed to have any type aggression problem or bite history.
Owner hereby waive and release Ryan Butalid, Release The Leash Dog Training hereinafter referred to as the "Training Organization", its employees, officers, members and agents from any and all liability of any nature, for injury or damage which I or my dog may suffer, including specifically, but without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of such damage or injury while attending any training session, or any other function, of the Training Organization, or while on the training grounds or the surrounding area thereto.
In consideration of and as inducement to the acceptance of my application for training membership by this Training Organization, I hereby agree to indemnify and hold harmless this Training Organization, its employees, officers, members and agents from any and all claims, or claims by any member of any family or any other person accompanying me to any training session or function to the Training Organization, or while on the grounds or the surrounding area thereto as a result of any action by any dog, including my own.
Drop Off
Your dog will need to be on a leash/collar that the dog cannot slip out of when getting out of your car and onto our property, then to a staff member. It is possible your dog will be scared, nervous or excited and this can lead to your dog trying to escape the collar and possibly running into the road which is unsafe, so be sure you have your dog secure. NO DOG HARNESS. If Owner fail to follow this safety drop off protocol and dog slipped out from its restraint equipment such as collar, leash and etc, Owner hold Ryan Butalid, Release The Leash Dog Training harmless from any liability.
Turn-Over
Your dog will be dropped off and turned over on your place, we will work with you and your dog to show you the type of work and training that took place while your dog was with us for Board and Train. We will also explain how you are to continue implementing the training into you and your dog's everyday life. You have unlimited time for this, we will not leave until you have crystal clear understanding how to handle your dog.
