    Please fill in the required fields and submit this form to complete your uniform order.

  • All newly endorsed TAFs from the 2023 season onwards will be issued with a TAF uniform kit including 1 x shirt, 1 x shorts and 1 x hat as part of the SLSNSW endorsement process.

    SLSNSW is transitioning from specific Trainer, Assessor or Facilitator shirts to general "Education" shirts. TAFs who gain an additional TAF award (such as a current Trainer now becoming an Assessor, or a current Assessor becoming a Facilitator) will receive the relevant new shirt if stock is available and will otherwise be supplied with an Education shirt.

  • Before placing your order, please refer to the following sizing charts - noting that in particular the women's style shorts are small sizes and most members have needed to order at least two sizes up from their usual size.

    Once submitted:  

    • Your selected uniform items will be delivered to your branch office

    • You will be notified by your Branch Director of Education when your order is ready for collection

  • Should be Empty:
