I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Sacred Pawz dog fostered by me.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Sacred Pawz dog rescue and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster dog.

I acknowledge that my personal dogs in my home must be up to date on all vaccines.

I accept full responsibility for the dog(s) actions at all times, and release Sacred Pawz dog rescue from any liabilities or damages that may be incurred because of fostering such dog(s).

I agree to notify Sacred Pawz immediately if the foster dog is lost, or out of my care including staying with someone else or a boarding facility.

I acknowledge that a form of Id will be provided to me for the foster dog and it must remain on the dog at all times.

I understand that I must contact Sacred Pawz immediately if the foster dog becomes ill, or injured, and that approval for medical care must be authorized by Sacred Pawz. In the event of an extreme medical emergency, go to the nearest Veterniary hospital in your area and contact Sacred Pawz.

By sending this electronically, I acknowledge that I have completely read this questionnaire and comprehend it fully.

I agree to have Sacred Pawz rescue complete reference call checks and conduct a home check to be able to approve my foster application.

I agree that if I'm unable to foster the dog(s) anymore that I will return the dog(s) to Sacred Pawz dog rescue and try to give Sacred Pawz dog rescue  a 2 week period to try and find another suitable foster for the dog(s).
