Policy C.08 Reinstatement Request Form
Instructions:
In accordance with BC Hockey Policy C.08 Section II b:

Anyone who participates in a non-sanctioned game after the Cut-off date (Sept 30) may seek early reinstatement of their eligibility for sanctioned BC Hockey programs by applying for such reinstatment based on there being special circumstances. BC Hockey shall return complete discretion in terms of garnting, or not, early reinstatment if satisfed that special circumstances exist.

2. A letter of position from the participant's applicable member (e.g. MHA, team or program)
3. A letter of position from the applicable regional Member (e.g. District MHA or League)
