CCDN Partnership Application Form
Thank you for your inquiry to partner with Community Care Development Network. This is the first step to partner. We ask all of our partners/potential partners to complete this form to help us better understand the connection and how we can mutually serve one another in our missions and the community.

If you are requesting a community food hub at your location we will:
- conduct a site visit to review operations and set up
- assist with orientation for your food hub coordinator and volunteers
Community food hub partnerships must have a minimum of 5 volunteers to help with the following:
- Administration
- Organization
- Set up and breakdown
- Loading and unloading food
Please note there is an administration fee associated with produce and with snack packs. Administrative fees may range between $2.00-$4.00 per bag of produce and snackpack.
