    FIRE PAC ENROLLMENT FORM

  • If you wish to contribute to the Local 1159 Fire PAC, please complete the information below. This allows a deduction to be made from your paycheck to be deposited in the Local 1159 FIRE PAC account. All funds contributed to the Local 1159 Fire PAC will only be used in non-partisan races at the fire board, city or county level. These funds will not contribute to partisan races or at state or federal level politics.

  • By affixing my signature below, I hereby authorize Local 1159, IAFF, to deduct the indicated amount from my payroll and to remit that amount to the IAFF Local 1159 FIREPAC fund. This authorization is signed voluntarily based on my specific understanding that: The FIREPAC uses the money they receive for political purposes including but not limited to making contributions to and expenditures for candidates and measures for city, county and local politics. I understand that the signing of this authorization and the making of these voluntary contributions are not conditions of membership in our Union or of employment by my employer. I also understand that I may refuse to contribute without reprisal, and that contributions to FIREPAC are not deductible as charitable contributions for Federal Income Tax purposes.

  • If you wish to contribute to the IAFF Fire PAC, please complete the information below.

  • I hereby authorize and direct you as my employer to deduct from my pay for the sum and pay periods detailed above and remit that amount to IAFF FIREPAC. This authorization shall remain in full effect until revoked by me in writing and is made voluntary upon my specific understanding that:

    1. The signing of this authorization card and the making of these voluntary contributions are not conditions of membership in the union or of employment by my employer;
    2. I may refuse to contribute without reprisal;
    3. And that FIREPAC, which is connected with the International Association of Fire Fighters, uses money received for political purposes, including, but not limited to making direct contributions to candidates, conducting expenditures on behalf of candidates and addressing political issues of public importance.
    4. The suggested contribution amounts below are merely suggestions and you may contribute more or less than those amounts and the IAFF will not favor or disadvantage anyone by reason of the amount of their contribution or their decision not to contribute.
    5. I declare that this contribution is given freely and voluntarily from my personal funds and I have not been compensated or reimbursed for this contribution.
