Waiver And Release

I/We hereby apply for enrolment of my child to the Art Academy at Art-Kitect Inc.. I have read the “Waiver And Release” form and clearly understand the terms and consent to the conditions. I/We hereby give the Art Academy at Art-Kitect Inc. and it’s employees permission to act on my/our behalf in case of emergency. I/We have disclosed all medical issues and other issues or notes that the instructors should need. I/We understand there may be inherent risk involved in any classes that my child may attend and I/We hereby release The Art Academy at Art-Kitect (Art-Kitect Conservation Inc.) and its teachers from liability in case of accident or injury. I understand professional instructors will conduct all classes in the safest way possible. I/We understand that the Art Academy at Art-Kitect will be taking photography and video for promotional use only and that my child may be within view and used in Art-Kitect’s advertisements and not sold.
I agree to what is stated in the "Waiver and Release" in the above paragraph and abide by the "Terms and Conditions" outlined official documents.*
PAYMENT

Once your registration form has been submitted an Art-Kitect staff member will process your form to confirm you're enrolled in the correct class or week of camp. We will then send you a welcome email with an invoice you may pay online. Cash payments will recieve a discount.

Once your invoice is paid, your spot is secured and you are confirmed for your classes or week of camp.
