    RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, INDEMNITY, AND PARENTAL CONSENT AGREEMENT ("AGREEMENT")

    IN CONSIDERATION of being permitted to participate in any way in ĒVLV LLC (ĒVLV) activities, including, without limitation, camps and clinics, leagues, parties, group events, games, soccer, futsal ("Activities"), at the ĒVLV space within the SCOR facility located at 1385 Overbrook Road, Richmond, Virginia 23220, or any other location, I, for myself and for my personal representatives, assigns, heirs, and next of kin, and for any minors identified above:

    AND I, THE MINOR’S PARENT AND/OR LEGAL GUARDIAN, ACKNOWLEDGE, AGREE, AND REPRESENT THAT I UNDERSTAND THE NATURE OF THE ACTIVITIES AND THE MINOR’S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH ACTIVITIES. I FURTHER AGREE AND WARRANT THAT IF AT ANY TIME I BELIEVE CONDITIONS TO BE UNSAFE, I WILL IMMEDIATELY DISCONTINUE THE MINOR’S FURTHER PARTICIPATION IN THE ACTIVITY. I ACKNOWLEDGE AND FULLY UNDERSTAND THAT: (a) THE ACTIVITIES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND DEATH ("RISKS"); (b) these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others participating in the Activity, including the minor, the condition of the facility or location in or at which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; (c) there may be OTHER RISKS AND LOSSES either not known to me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my participation or that of the minor in the Activity. I HEREBY RELEASE AND FOREVER DISCHARGE EVLV LLC., and each of their shareholders, directors, officers, administrators, agents, members, volunteers, contractors, and employees, all other participants, any sponsors, advertisers, and, if applicable, any other owner or lessor of premises on which the Activity takes place (each considered one of the "RELEASES" herein) from, AND COVENANT NOT TO SUE RELEASES for, ALL LOSSES, COSTS, CLAIMS, DEMANDS, EXPENSES, CAUSES OF ACTION, LOSSES OF USE, AND DAMAGES I or the minor may have, or that may hereafter accrue to me or the minor, as a result of the minor’s participation in the Activities. I AGREE TO INDEMNIFY, DEFEND, AND HOLD SCOR AND ALL OTHER RELEASES HARMLESS from any and all loss, costs, claims,

    demands, expenses, causes of action, losses of use, damages, and liability by reason of injury (including death) to persons or damage to property caused by, arising out of, or resulting from the minor’s participation in the Activities, irrespective of who or what may be the cause of such loss, cost, claim, demand, expense, cause of action, loss of use, damages, or liability. In the event of an EMERGENCY OR NON-EMERGENCY SITUATION REQUIRING MEDICAL TREATMENT, I hereby grant permission for any and all medical and/or dental attention to be administered to my child/children, in the event of illness or an accidental injury, until such time as I can be contacted. This permission includes, but is not limited to, THE ADMINISTRATION OF FIRST AID, THE USE OF AN AMBULANCE, TRANSPORT TO AN AREA HOSPITAL, AND THE USE OF ANESTHESIA AND SURGERY UNDER THE RECOMMENDATION OF QUALIFIED MEDICAL PERSONNEL. Any term or provision of this Agreement which is invalid or unenforceable in any jurisdiction will, as to that jurisdiction, be ineffective to the extent of such invalidity or unenforceability without rendering invalid or unenforceable the remaining terms and provisions of this Agreement or affecting the validity or enforceability of any of the terms or provisions of this Agreement in any other jurisdiction. If any provision of this Agreement is so broad as to be unenforceable, the provision will be interpreted to be only so broad as is enforceable. This Agreement shall be governed and construed in accordance with the laws of the Commonwealth of Virginia, except for Virginia’s choice of law statutes. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE, INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW, AND.

  • MEDIA RELEASE

    I, the undersigned, do hereby consent and agree that ĒVLV LLC, its employees, or agents, have the right to take photographs, videotape or digital recordings of my child(ren) listed on this registration form and to use these in any and all media, now or hereafter known, and exclusively. I further consent that my child(rens) name and identity may be revealed therein or by descriptive text or commentary. I do hereby release to ĒVLV LLC, its agents and employees all rights to exhibit this work in print and electronic form publicly or privately and to market and sell copies. I waive any rights, claims or interest I may have to control the use of my identity or likeness in whatever media used. I understand that there will be no financial or other remuneration for recording me or my child(ren), either for initial or subsequent transmissions or playback. I also understand that ĒVLV LLC is not responsible for any expense or liability incurred as a result of my child(ren) participation in this recording, including medical expenses due to any sickness or injury incurred as a result.

  • Concussion Information and Consent Form


    A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly. In other words, even a “ding” or a bump on the head can be serious. You can’t see a concussion and most sports concussions occur without loss of consciousness. Signs and symptoms of concussion may show up right after the injury or can take hours or days to fully appear. If your child reports any symptoms of concussion, or if you notice the symptoms or signs of concussion, seek medical attention right away.

    Symptoms may include one or more of the following:

    Headaches
    Amnesia
    “Pressure in head” “Don’t feel right” Nausea or vomiting Fatigue or low energy Neck pain

    Sadness
    Balance problems or dizziness Nervousness or anxiety Blurred, double, or fuzzy vision
    Sensitivity to light or noise
    More emotional Feeling sluggish or slowed down
    Confusion
    Feeling foggy or groggy
    Concentration or memory problems Drowsiness (forgetting game plays) Change in sleep patterns

    Signs observed by teammates, parents and coaches include: Slurred speech
    Appears dazed
    Vacant facial expression
    Confused about assignment
    Forgets plays
    Is unsure of game, score, or opponent Moves clumsily or displays in coordination Answers questions slowly
    Shows behavior or personality changes Can’t recall events prior to hit
    Can’t recall events after hit
    Seizures or convulsions

    Any change in typical behavior or personality
    Loses consciousness

    Athletes with the signs and symptoms of concussion should be removed from play immediately. Continuing to play with the signs and symptoms of a concussion leaves the young athlete

    especially vulnerable to greater injury. There is an increased risk of significant damage from a concussion for a period of time after that concussion occurs, particularly if the athlete suffers another concussion before completely recovering from the first one. This can lead to prolonged recovery, or even to severe brain swelling (second impact syndrome) with devastating and even fatal consequences. It is well known that adolescent or teenage athletes will often under report symptoms of injuries. And concussions are no different. As a result, education of administrators, coaches, parents and students is the key for student athlete’s safety.

    Any athlete even suspected of suffering a concussion should be removed from the game or practice immediately. No athlete may return to activity after an apparent head injury or concussion, regardless of how mild it seems or how quickly symptoms clear, without medical clearance. Close observation of the athlete should continue for several hours.

    You should also inform your child’s coach if you think that your child may have a concussion. Remember, it’s better to miss one game than miss the whole season. And when in doubt, the athlete sits out.

  • COVID-19 Liability Waiver

    I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that ĒVLV LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that ĒVLV LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, training staff, and other clients and their families. I voluntarily seek services provided by ĒVLV LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all ĒVLV LLC or SCOR procedures to reduce the spread while participating in all ĒVLV LLC activities.

    I attest that:
    * I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty

    breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
    * I have not traveled internationally within the last 14 days.
    * I have not traveled to a highly impacted area within the United States of America in the last 14 days.

    * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
    * I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

    * I am following all CDC recommended guidelines as much as possible and limiting my exposure

    to the Coronavirus/COVID-19.

    I hereby release and agree to hold ĒVLV LLC harmless from, and waive on behalf of myself, my

    heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that

    may be caused by any act, or failure to act of the business or that may otherwise arise in any way in connection with any services received from ĒVLV LLC. I understand that this release discharges ĒVLV LLC from any liability or claim that I, my heirs, or any personal representatives may have against the business with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from ĒVLV LLC. This liability waiver and release extends to the business together with all owners, partners, employees, and volunteers.
