By signing I understand that all deposits left with The Fox & Fable are nonrefundable! Deposits are able to be rolled over if you give at least 72 hours notice to give us ample time to fill your spot! If not I agree to forfeit my deposit amount. By signing I also understand that appointment dates may change on the artist ends at any time due to last-minute doctors appointments/travel dates and that my deposit will roll over onto the reschedule date discussed. By signing I understand that the deposit amount is the artists drawing fee and is non refundable. I also understand that no changes can be made to the design after submitting this form. By signing I understand that visitors are not allowed into the studio at all and under no circumstances. You will need to come to your appointment alone. If you have diabetes or any skin conditions please be sure to bring in a doctors note stating that it is OK for you to be able to get tattooed with your condition. I also understand that I am being tattooed by tattoo apprentices who are still in the learning stages of being tattooed.
