    Tenant Contact & Emergency Information/Release of Liability - Lessee

  • Both Sections 1 & 2 of this form must be completed and signed by the tenants/lessees.

  • Section 1: Tenant Contact & Emergency Information

  • In case of an emergency, the Association will attempt to contact the tenant or homeowner before the emergency contacts listed below. The third option will be for the Association to handle the emergency in the best interests of the homeowner and the neighborhood. That may include incurring costs to secure the lot/unit from further damage and future safety issues.

  • It is our responsibility as tenants to make sure this information is kept current and accurate. I/We also agree that the Association may take measures to secure the lot/unit (which may incur costs that will be the responsibility of the homeowner) if all of the contacts listed above are unavailable.

    All information contained in this document is for the exclusive use of the Eagle Rock Homeowners Association.

  • Section 2: Release of Liability - Lessee

  • I/We, the undersigned who currently are leasing property in Eagle Rock at Snow Canyon, in consideration of the Homeowners Association (HOA)/homeowner allowing use of the common areas located at 250 Snow Canyon Drive, Ivins, Utah, agree to defend and hold the HOA and its officers, directors, shareholders, employees, agents, and successors harmless from and against any and all claims, liability, damages, costs, and losses of whatever kind which may arise out of being in any way connected with the use of the common areas at Eagle Rock by me, my guests, invitees, agents, or third parties who have access to the common areas through me, except for such claims which arise out of gross negligence or intentional misconduct of the HOA. I/We acknowledge that all family members and guests have been informed of, and will abide by, the most current HOA Rules and Regulations. Failure to abide by the HOA Rules and Regulations will result in a potential fine to the homeowner and a loss of privileges for a period of time as delineated in the Schedule of Fines.
