**All scheduling is done with Coach Rachel (571-331-6249) directly. Please text or call me to scheduel a time for your child(ren).** 

Registration fee:

$100 for 1 child or if registering more than 1 child together, $50 additional per additional child. This is a ONE TIME fee, no annual fees here.

Weekly Tuition:

$110/week per child

Please check your vacation time, school, and appointments schedule before registering for swim lessons. 

Classes are 4 days per week, Monday through Thursday and are 10 minutes per lesson ONE ON ONE with the coach. Lessons are offered between 8:00am & 6:00pm. **Please contact Rachel directly for her hours. You will keep your scheduled time for the remainder of the lessons.

Registration fee MUST be paid with form submission in order to secure a time slot. No time slots will be held with the form alone. Fee can be paid via Venmo to @Survivalswimforlittles, ApplePay or Zelle to 571-331-6249. Registration is NON-REFUNDABLE. Failure to receive payment may result in your timeslot(s) to be forfeited.

SECTION 1 - RELEASE OF LIABILITY

I, the undersigned, in consideration for myself and/or my minor child being permitted to participate in lessons provided by Ms. Rachel Survival Swim for Littles, LLC, hereby release and hold harmless Ms. Rachel Survival Swim for Littles, LLC, its owners, operators, instructors, pool hosts, employees, agents, and volunteers (“Released Parties”) from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child while in or upon the premises, under the supervision of the Released Parties, or while traveling to or from such premises, to the fullest extent permitted by Florida law.

I further acknowledge and agree that:

• Participation in swimming or survival swim lessons carries inherent risks, including the risk of drowning, even when reasonable care is exercised.
• Enrollment, participation, or completion of lessons does not “drown-proof” a child or guarantee water safety.
• As parent or legal guardian, I am ultimately responsible for my child’s safety in and around water and must ensure vigilant, continuous supervision at all times, regardless of swimming ability.

NOTICE TO THE MINOR CHILD’S NATURAL GUARDIAN

READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF MS. RACHEL SURVIVAL SWIM FOR LITTLES, LLC USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD’S RIGHT AND YOUR RIGHT TO RECOVER FROM MS. RACHEL SURVIVAL SWIM FOR LITTLES, LLC IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND MS. RACHEL SURVIVAL SWIM FOR LITTLES, LLC HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.

 

SECTION 2 – ASSUMPTION OF RISK

I understand, acknowledge, and agree that swimming, diving, and related water activities involve inherent risks that cannot be eliminated even when reasonable care is used. I further acknowledge that these activities may be strenuous and physically demanding.

To the best of my knowledge, the participant(s) I am registering have no medical, physical, or emotional condition that would hinder or prevent safe participation in programs offered by Ms. Rachel Survival Swim for Littles, LLC. I accept full responsibility for consulting a physician regarding any condition that may affect safe participation.

 

SECTION 3 – INDEMNIFICATION 

I agree to indemnify, defend, and hold harmless the Released Parties against any and all claims, demands, causes of action, damages, costs, or expenses (including reasonable attorney’s fees) that may arise from or relate to my or my child’s participation in lessons or use of the facilities, except to the extent caused by the gross negligence or intentional misconduct of the Released Parties.

 

SECTION 4 – MEDICAL CONSENT

In the event of injury, accident, or illness during participation, I authorize the Released Parties to obtain or arrange any emergency medical treatment deemed necessary for myself or my child. I accept full financial responsibility for any such treatment and related costs, including transportation.

Enrollment review and payment policies: I understand that I am enrolling for swim lessons with Ms. Rachel Survival Swim for Littles and that the information submitted on this enrollment form full require review as it is the intent of Ms. Rachel Survival Swim for Littles' to provide each participant with an individualized, effective and safe learning experience. I acknowledge that in some health related cases, Ms. Rachel Survival Swim for Littles may not be able to provide services. I certify that all medical/health information I have provided is correct and true and agreed to provide additional information, including a physicians clearance if requested, prior to the beginning of any lessons. I understand that in order to secure my child’s selected lesson time, I must pay all applicable registration fees per the instructions and sent to me. I understand that participant enrollment may be canceled if payment is not received. *

Once this registration form is submitted, you agree to Venmo @Survivalswimforlittles or Zelle or Apple Pay to 571-331-6249 the registration fee. **Please note NO timeslot(s) are saved without both form AND fee. 

 

**Weekly lesson tuition is then due the FRIDAY BEFORE the Monday your child(ren) will be in the water.**
